血清白细胞介素-17在首发抑郁障碍患者中的表达及其影响因素研究

摘　　要：目的探讨血清白细胞介素(IL)-17在首发抑郁障碍患者中的表达,并在当前医学模式下研究其心理社会影响因素。方法病例组在药物治疗前后采用酶联免疫吸附法(ELSIA)测定血清IL-17水平,对照组仅测1次。病例组在治疗前运用汉密尔顿抑郁量表(HAMD)、生活事件量表(LES)、埃森克人格问卷(EPQ)、社会支持评定量表(SSRS)进行心理测查。对数据进行描述性统计分析,统计学方法采用t检验、χ2检验及多元线性回归分析。结果血清IL-17浓度与抑郁程度呈正相关(r=0.38,P<0.05);病例组治疗前血清IL-17显著高于对照组(t=25.00,P<0.05),治疗后显著低于治疗前(t=-14.35,P<0.05);血清IL-17的重要社会影响因素依次为:LES负性、掩饰、性别、内外向、客观支持、年龄和精神质。结论在首发抑郁障碍患者的病程中,血清IL-17起着重要作用,且与心理社会因素存在一定关联。Objective To investigate the expression of serum IL-17 and its psychosocial influencing factors related to current medical model,in patients with first-episode major depressive disorders(MDD).Methods The serum IL-17 level was measured by the enzyme-linked immunosorbent assay(ELSIA)in the MDD patients before and after medication,whereas the control group was tested only once.Before the treatment,the MDD group was psychologically evaluated with Hamilton depression scale(HAMD),life events scale(LES),Essenck personality questionnaire(EPQ),and social support rating scale(SSRS).SPSS22.0 software was used for descriptive statistical analysis,t test,χ2 test and multiple linear regression of the data.Results There was a significant positive correlation between the serum IL-17 level and severity of MDD(r=0.38,P<0.05).The serum IL-17 level in the MDD group was significantly higher than that in the control group at baseline(t=25.00,P<0.05),and was significantly reduced after treatment(t=-14.35,P<0.05).The psycosocial influencing factors of serum IL-17,in order of the correlation strength,were LES negativity,dissimulation,gender,introversion and extroversion,objective support,age and neuroticism.Conclusion Serum IL-17 plays an important role in the course of first-episode major depressive disorder,and is related to psychosocial factors.
