综合性早期干预对极低出生体重早产儿身体和智力发育的影响

摘　　要：目的探讨综合性早期干预对极低出生体重早产儿(VLBW)身体和智力发育的影响。方法选择2013年4月~2017年12月我院产科出生的VLBW 72例,采用抛银币法将72例VLBW分为观察组(n=36)和对照组(n=36)。两组VLBW均予以常规干预措施,观察组VLBW在此治疗基础上予以综合性早期干预。两组VLBW均干预12个月。观察并比较观察两组VLBW干预12个月后身体和智力发育情况。结果干预6个月和12个月后,两组VLBW身高、体重和头围均较干预前明显上升(P<0.05或P<0.01),且观察组上升的幅度较对照组更明显(P<0.05);干预12个月后,观察组VLBW精细运动、适应、语言、社交和大运动等智能发育商评分均明显高于对照组(P<0.05)。结论综合性早期干预不仅有助于VLBW的身体发育,促进身高、体重和头围的追赶性生长,而且有助于VLBW运动及智力的发育,更好地改善VLBW预后,保障其健康成长。Objective To investigate the influence of comprehensive early intervention on the physical and mental development of very low birth weight premature infants(VLBW).Methods Seventy-two VLBW born in the hospital from April 2013 to December 2017 were enrolled and divided into the observation group(n=36)and the control group(n=36)by the silver coin throwing method.Both groups were given routine intervention measures,on the basis of which the observation group was given additional comprehensive early intervention.Both groups were intervened for 12 months.The physical and mental development of the two groups after 12 months of intervention was observed and compared.Results After 6 months and 12 months of intervention,the height,weight and head circumference of the two groups were significantly higher than those before intervention(P<0.05 or P<0.01),and the increase of the observation group was more obvious than that of the control group(P<0.05).Meanwhile,after 12 months of intervention,the scores of intelligent development quotients such as fine movement,adaptation,language,social contact and large exercise of the observation group were significantly higher than those of the control group(P<0.05).Conclusion Comprehensive early intervention not only contributes to the physical development of VLBW and promoting the catch-up growth of height,weight and head circumference,but also contributes to their movement and intelligence development,improving the prognosis of VLBW and ensuring their healthy growth.
