危重症患者失禁性皮炎的发生现况及其影响因素

摘　　要：目的研究探讨危重症患者发生失禁性皮炎的情况以及影响其发生的因素。方法选择2017年1月~2018年12月在我院接受治疗的危重症并伴有失禁的患者70例作为临床研究对象进行回顾性分析,观察其发生失禁性皮炎的情况,并采用Logistic回归分析导致失禁性皮炎发生的相关因素。结果通过分析,镇静剂使用情况、抗生素使用情况、营养支持方式、是否合并基础疾病、体温、体重、Barthel指数以及性别无显著差异(P>0.05);会阴部位皮肤状况评分、大便性状、失禁类型、进行机械通气与否、感染存在与否、血清白蛋白水平、营养风险筛查量表评分、Braden压疮量表以及意识障碍存在与否情况具有统计学意义(P<0.05);血清白蛋白水平属于保护性因素,意识障碍、大便性状以及失禁类型属于该疾病的独立危险因素。结论在危重症患者治疗中,可根据分析结果在患者的治疗期间给予相应的护理干预,使患者皮肤完整性得以维持,降低失禁性皮炎的发生率,为患者的生活质量做出保障。Objective To investigate the occurrence of incontinence dermatitis in critically ill patients and the factors affecting its occurrence.Methods 70 cases of critically ill patients with incontinence who were treated in our hospital from January 2017 to December 2018 were retrospectively analyzed.The occurrence of incontinence dermatitis was observed.Logistic regression analysis was used to analyze the related factors leading to the occurrence of incontinence dermatitis.Results Through analysis,there were no significant differences in sedative use,antibiotic use,nutrition support methods,whether to combine underlying diseases,body temperature,weight,Barthel index,and gender(P>0.05).There were significant differences in perineal skin condition score,stool characteristics,type of incontinence,presence or absence of mechanical ventilation,presence of infection,serum albumin level,nutritional risk screening scale score,Braden pressure ulcer scale,and presence or absence of consciousness(P<0.05).Serum albumin levels were protective factors,and disorders of consciousness,stool characteristics,and types of incontinence were independent risk factors for the disease.Conclusion In the treatment of critically ill patients,corresponding nursing interventions can be given during the treatment of patients according to the analysis results,to maintain the skin integrity of the patients,reduce the incidence of incontinence dermatitis,and to guarantee the quality of life of patients.
