医护助一体化协作模式在内分泌科门诊中的应用

摘　　要：目的探讨医护助一体化协作模式的构建在内分泌科门诊工作中的作用。方法构建医护助一体化协作模式(设置巡视及健康教育护士和专职医生助理),比较实施医护助一体化协作模式前后,患者获得的医疗服务时间、患者候诊时间、再次返回诊间人数、医生看诊量以及患者与医生满意度评分。结果实施医护助一体化协作模式后,患者获得医疗服务时间由5.3min延长至6.5min;患者的候诊时间由26.4min缩短至22.5min;患者再次返回诊间人数由医生每次看门诊期间(以下简称每诊)8.1人次减少至每诊2.9人次;6位内分泌科医生每诊门诊量由平均39.7人次增加至46.6人次;患者满意度评分由(92.0±5.7)分增加至(97.6±2.3)分,有统计学差异(P<0.01);医生满意度评分由(87.7±2.9)分上升至(96.3±3.2)分,有统计学差异(P<0.01)。结论实施医护助一体化协作模式可提高内分泌科门诊医生的工作质量和效率,提升护士及医生助理的知识水平和沟通能力,改善患者的就医体验,值得推广。Objective To explore the role of integrated physician-nurse-physician assistant collaboration mode in endocrinology outpatient. Methods The integrated physician-nurse-physician assistant collaboration mode(rounding nurse, health education nurse and full-time physician assistant were arranged) was established and the duration of medical services, the waiting time, the number of round trips to consulting room, the number of patient visits and the satisfaction score of patients and physicians before and after the application of the mode were compared. Results After application of the integrated physician-nurse-physician assistant collaboration mode, the duration of medical services was extended from 5.3 min to 6.5 min, the waiting time was shortened from 26.4 min to 22.5 min, the number of round tri ps to consulting room was reduced from 8.1 visits to 2.9 visits, the mean number of patient visits was increased from 39.7 visits to 46.6 visits according to the outpatient results of six endocrinologists, and the satisfaction score of patients and physicians was dramatically elevated, from 92.0±5.7 to 97.6±2.3(P < 0.01) and from 87.7±2.9 to 96.3±3.2(P < 0.01). Conclusion The integrated physician-nurse-physician assistant collaboration mode can improve the work quality and efficiency of the physicians in the endocrinology outpatient, enhance the professional knowledge and communication ability of nurses and physician assistants, and optimize clinic experience of patients. So the mode deserves to be popularized.
