摘  要:目的分析比较高胆固醇(TC)脂血症患者血清小而密低密度脂蛋白(sdLDL)、低密度脂蛋白(LDL)、甘油三酯(TG)、TC水平,观察sdLDL与LDL、TC的关联性。方法随机收集我院2019年7-9月1238例行血脂常规检查人群,根据TC水平分为实验组(高TC患者417例,其中238例高TC伴高TG患者为实验Ⅰ亚组,179例单独高TC患者为实验Ⅱ亚组)和对照组(TC水平正常人群821例)。研究比较两组(含2个实验亚组)sdLDL、LDL、TG、TC水平结果,观察sdLDL、LDL敏感度、特异度、符合率的特点和规律;采用ROC曲线分析评估sdLDL对高脂血症TC的表达准确性;采用Pearson相关分析评估sdLDL与TC、LDL相关性(γ表示相关系数)。结果实验组(含2个亚组)sdLDL和LDL水平均显著高于对照组(P<0.001);实验组sdLDL及LDL阳性例数均明显高于对照组,而对照组sdLDL及LDL阴性例数均明显高于实验组,其分布均存在显著差异(P<0.005),sdLDL及LDL均与TC有显著相关(P<0.001),sdLDL的敏感度和特异度及符合率均接近LDL;ROC曲线下面积AUC=0.896,sdLDL能显著反映TC水平(有统计学意义,P=0.000);实验组(含2个亚组)和对照组sdLDL与TC、LDL均有显著相关(P<0.001),实验组(含2个亚组)和对照组的sdLDL与LDL均有显著相关(P<0.001)。结论sdLDL与LDL、TC有显著关联,sdLDL可用于高TC脂血症临床监测,为ASCVD调脂治疗监测提供思路和方法。Objective To analyze and compare the serum levels of small and dense low-density lipoprotein(sdLDL),low-density lipoprotein(LDL),total cholesterol(TC),triglycerides(TG)with Hypercholesterolemia,observe the correlations between sdLDL and TC or LDL.Reveal the clinical value of sdLDL in Hypercholesterolemia,lay a foundation in the prevention and treatment of Arteriosclerosis cardiovascular disease(ASCVD).Methods Selected randomly 1238 patients by routine examination of blood lipid in our hospital from July to September 2019,they were divided into experimental group(417 cases with high TC,including 238 cases the experimental subgroupⅠwith high TC and high TG,179 cases the experimental subgroupⅡwith only-high TC)and control group(821 cases with normal TC)by TC levels.Compare and study results of sdLDL,LDL,TC and TG in the two groups(including two subgroups),observe the characteristics and rules of sdLDL and LDL in sensitivity,specificity and coincidencerate;ROC curve analysis was used to evaluate the expression accuracy of sdLDL on TC in hyperlipidemia;Pearson correlation analysis was used to evaluate the correlation between sdLDL and TC or LDL(γis the correlation coefficient).Results The levels of sdLDL and LDL in the experimental group(including two subgroups)were significantly higher than those in the control group(P<0.001);The positive numbers of sdLDL or LDL in the experimental group were significantly higher than those in the control group,while,the negative cases number of sdLDL or LDL in the control group were significantly higher than those in the experimental group,both sdLDL and LDL were related to TC with significant differences in their distribution(P<0.005),sdLDL and LDL were significantly correlated to TC(P<0.001),sdLDL in Sensitity,specificity and coincidencerate was similar to LDL;Area under ROC curve AUC=0.896,sdLDL can significantly reflect TC level(P=0.000).sdLDL had significant correlation with TC or LDL between the experimental group(including two subgroups)and the control group(P<0.001),sdL
