头孢丙烯联合喜炎平应用在急性化脓性扁桃体炎患儿治疗中对PCT、TNF-α及IL-6水平的影响

摘　　要：目的分析头孢丙烯联合喜炎平应用在急性化脓性扁桃体炎患儿治疗中对降钙素原(PCT)、肿瘤坏死因子-α(TNF-α)及白细胞介素-6(IL-6)水平的影响。方法359例急性化脓性扁桃体炎患儿,按分层随机法分为观察组(254例)和对照组(105例)。对照组患儿采用头孢丙烯治疗,观察组患儿在对照组基础上联合喜炎平治疗。对比两组症状消失时间、临床疗效、不良反应发生情况及治疗前后PCT、IL-6和TNF-α水平。结果观察组发热、咽痛和脓性分泌物消失时间均短于对照组,差异具有统计学意义(P<0.05)。治疗前,两组PCT、IL-6和TNF-α水平对比差异无统计学意义(P>0.05);治疗后,两组PCT、IL-6和TNF-α水平均低于本组治疗前,且观察组低于对照组,差异具有统计学意义(P<0.05)。观察组治疗总有效率为97.24%,高于对照组的79.05%,差异具有统计学意义(P<0.05)。观察组不良反应发生率为4.33%,低于对照组的32.38%,差异具有统计学意义(P<0.05)。结论在急性化脓性扁桃体炎中联合头孢丙烯和喜炎平治疗可缩短患儿症状消失时间,消除炎症反应,提高疗效和安全性,值得应用。Objective To analyze the effect of cefprozil combined with Xiyanping on the levels of procalcitonin(PCT),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in the treatment of patients with acute suppurative tonsillitis.Methods A total of 359 child patients with acute suppurative tonsillitis were divided into observation group(254 cases)and control group(105 cases)by stratified randomization.The control group was treated by cefprozil,and the observation group was treated by Xiyanping on the basis of the control group.The symptom disappearance time,clinical efficacy,occurrence of adverse reactions and levels of PCT,TNF-αand IL-6 before and after treatment were compared between the two groups.Results The disappearance time of fever,pharyngalgia and purulent secretion of the observation group were shorter than those of the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in levels of PCT,TNF-αand IL-6 between the two groups(P>0.05).After treatment,the levels of PCT,TNF-αand IL-6 of the two groups were lower than those before treatment of the same group,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05).The total effective rate of treatment of the observation group was 97.24%,which was higher than that of the control group 79.05%,and the difference was statistically significant(P<0.05).The incidence of adverse reactions of the observation group was 4.33%,which was lower than that of the control group 32.38%,and the difference was statistically significant(P<0.05).Conclusion The combination of cefprozil and Xiyanping in acute suppurative tonsillitis can shorten the disappearance time of patients’symptoms,eliminate the inflammatory response,improve the efficacy and safety,which is worth applying.
