早期预警性护理干预对高龄股骨粗隆间骨折患者应激反应及焦虑抑郁情绪的影响

摘　　要：目的探讨早期预警性护理干预对高龄股骨粗隆间骨折患者应激反应及焦虑、抑郁情绪的影响.方法将78例高龄股骨粗隆间骨折患者按照随机数字表法分为研究组与对照组,各39例.两组均给予常规护理干预,研究组在此基础上给予早期预警性护理干预,观察住院全程.比较两组术前及术中心率、血压、呼吸频率,干预前后采用焦虑自评量表及抑郁自评量表评定两组焦虑、抑郁情绪,比较两组术后并发症发生率及护理满意度.结果 术中研究组心率、血压、呼吸频率与术前比较差异无统计学意义(P>0.05),对照组显著高于术前且显著高于研究组(P<0.01).干预后两组焦虑自评量表及抑郁自评量表评分均较干预前显著降低(P<0.01),研究组较对照组下降更显著(P<0.01).研究组术后并发症发生率显著低于对照组(P<0.05),护理满意率显著高于对照组(P<0.05).结论早期预警性护理干预可减轻高龄股骨粗隆间骨折患者术中的应激反应,缓解焦虑、抑郁情绪,降低并发症发生率,提高护理满意度.Objective To explore the effect of early warning nursing intervention on intraoperative physio­logical stress response in elderly patients with intertrochanteric fracture.Methods A total of 78 patients with intertrochanteric fractures were divided into research group and control group according to random number table method,39 cases each.Both groups were given routine nursing intervention,and the re­search group was given early warning nursing intervention on this basis.Preoperative and surgical center rate,blood pressure,respiratory rate between the two groups were compared.Before and after the inter­vention,SAS and SDS were used to assess the anxiety and depression of the two groups,and the postoper­ative complications and nursing satisfaction were compared between the two groups.Results There was no statistically significant difference in heart rate,blood pressure,respiratory rate between the study group and before operation(P〉0.05),the control group was significantly higher than before operation and significantly higher than the study group(P<0.01).After intervention,the SAS and SDS scores of the two groups were significantly lower than before intervention(P<0.01),and the study group de­creased more significantly than the control group(P<0.01).Conclusions Early warning nursing inter­vention can alleviate the stress response,relieve anxiety and depression in elderly patients with intertro­chanteric fractures,reduce the incidence of complications,and improve nursing satisfaction.
