米力农治疗缓慢型心律失常并难治性心力衰竭临床疗效

摘　　要：目的观察米力农治疗缓慢型心律失常并难治性心力衰竭的临床疗效。方法选择2017年1月-2018年1月广东省惠东县人民医院心血管内科收治的缓慢型心律失常并难治性心力衰竭患者40例,采用随机数字表法分为治疗组和对照组各20例。对照组使用常规药物治疗,治疗组在对照组基础上加用米力农治疗。比较2组临床疗效、治疗前后左室射血分数(LVEF)、左室短轴缩短率(LVFS)、左室舒张末内径(LVEDD)、N端B型脑钠肽前体(NT-proBNP)、心率、QT间期。结果治疗组总有效率为95.00%,高于对照组的65.00%(χ~2=5.625,P<0.05)。治疗后2组LVEF、LVFS、LVEDD及NT-proBNP均优于治疗前,且治疗组优于对照组(P<0.01)。2组治疗前后心率及QT间期比较差异均无统计学意义(P>0.05)。结论米力农治疗缓慢型心律失常并难治性心力衰竭在不影响患者心率的前提下,可增加心肌收缩力,减轻心脏前后负荷,改善心室重构。Objective To observe the clinical efficacy of milrinone in the treatment of chronic arrhythmia and refractory heart failure.Methods 40 patients with bradycardia and refractory heart failure admitted to the Department of Cardiology,People’s Hospital of Huidong County,Guangdong Province from January 2017 to January 2018 were selected.The patients were divided into treatment group and control group by random number table,each of 20 cases.The control group was treated with conventional drugs,and the treatment group was treated with milrinone on the basis of the control group.The clinical efficacy,before and after treatment left ventricular ejection fraction(LVEF),left ventricular short axis shortening rate(LVFS),left ventricular end-diastolic diameter(LVEDD),N-terminal B-type brain natriuretic peptide precursor(NT-proBNP),heart rate,QT interval were compared between the 2 groups.Results The total effective rate in the treatment group was 95.00%,which was higher than 65.00%in the control group(χ~2=5.625,P<0.05).After treatment,LVEF,LVFS,LVEDD,and NT-proBNP of the 2 groups were better than before treatment,and the treatment group was better than control group(P<0.01).There was no significant difference in heart rate and QT interval before and after treatment between the 2 groups(P>0.05).Conclusion Milrinone in treatment of chronic arrhythmia and refractory heart failure does not effect the patient’s heart rate,it can increase myocardial contractility,reduces pre-and post-heart load,improves ventricular remodeling.
