甲泼尼龙琥珀酸钠治疗重症哮喘并发呼吸衰竭的临床疗效

摘　　要：目的探讨甲泼尼龙琥珀酸钠治疗重症哮喘并发呼吸衰竭的临床疗效。方法选取莒南县人民医院2016年6月—2019年6月收治的重症哮喘并发呼吸衰竭患者40例,依据双盲法分为常规组与试验组,各20例。常规组予以常规治疗,试验组在常规组基础上予以甲泼尼龙琥珀酸钠治疗。两组均持续治疗1周。比较两组临床疗效,治疗前后肺功能指标〔包括用力肺活量(FVC)、第1秒用力呼气末容积(FEV1)及最大呼气流量(PEF)〕、血气指标〔包括动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)及pH值〕、心率(HR)、呼吸频率(RR),并观察两组不良反应发生情况。结果试验组临床疗效优于常规组(P<0.05)。治疗前两组FVC、FEV1、PEF、PaO2、PaCO2、pH值、HR、RR比较,差异无统计学意义(P>0.05);治疗后试验组FVC、FEV1、PEF、PaO2、pH值高于常规组,PaCO2、HR、RR低于常规组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论甲泼尼龙琥珀酸钠治疗重症哮喘并发呼吸衰竭的临床疗效确切,可有效改善患者肺功能、血气指标及生命体征,且安全性较高。Objective To explore the clinical efficacy of methylprednisolone sodium succinate in the treatment of severe asthma complicated with respiratory failure.Methods A total of 40 cases of patients with severe asthma complicated with respiratory failure were selected from June 2016 to June 2019 in Junan County People’s Hospital,which were divided into routine group and experimental group,20 cases in each group.The routine group was given routine treatment,the experimental group was treated with methylprednisolone sodium succinate on the basis of routine group.Both groups were treated continuously for 1 week.Clinical efficacy,lung function index(Including FV,FEV1,PEF)and blood gas index(including PaO2,PaCO2,pH value)before and after treatment,HR,RR were compared between the two groups,and the incidence of adverse reactions were observed.Results The clinical efficacy of the experimental group was better than routine group(P<0.05).Before treatment,there was no significant difference in FVC,FEV1,PEF,PaO2,PaCO2,pH,HR and RR between the two groups(P>0.05);after treatment,the experimental group of FVC,FEV1,PEF,PaO2 and pH value were higher than routine group,while PaCO2,HR and RR were lower than routine group(P<0.05).There was no significant difference in the incidence rate of adverse reactions between the two groups(P>0.05).Conclusion Methylprednisolone sodium succinate have an exact clinical effect for severe asthma complicated with respiratory failure,which can effectively improve the lung function,blood gas index and vital signs of patients,and with high safety.
