摘 要:目的评估三维重建软件(IQQA系统)应用于多发肺磨玻璃结节(GGO)患者同期行胸腔镜下肺段切除手术的临床应用价值。方法统计2018年5月1日~2019年10月1日期间符合纳入标准的GGO共计25例患者,根据IQQA使用情况,将患者分为两组:IQQA组15例、非IQQA组10例,比较两组患者的一般临床特点及手术情况。结果 IQQA智能系统均能重建出血管、支气管走形分支及变异情况,术中段、亚段级支气管、血管均与术前相符。IQQA组15例患者均在全胸腔镜下行解剖性肺段(亚段)切除术,非IQQA组有3例患者中转改行肺叶切除术,其余患者均行解剖性肺段切除术。两组患者在术中结节定位时间以及手术时间上差异显著(P<0.05)。结论 IQQA用于胸腔镜下肺段、亚肺段及联合亚段切除的病灶定位、术中导航安全可行。Objective To evaluate the clinical value of three-dimensional reconstruction software in patients with multiple ground glass nodules undergoing thoracoscopic pulmonary segmental resection.Methods Twenty-five GGO patients who met the inclusion criteria from May 1,2018 to October 1,2019 were selected and divided into two groups based on IQQA usage:15 in the IQQA group and 10 in the non-IQQA group.The general clinical features and surgical conditions of the two groups were compared.Results Three-dimensional reconstruction can reconstruct blood vessels,bronchial branch and variation.During operation,all levels of trachea,bronchial and blood vessels were consistent with those before operation.In the IQQA group,15 patients underwent total thoracoscopic anatomic pulmonary segment(subsegment) resection;in the non-IQQA group,3 patients in the non-IQQA group were transferred to lobectomy and the rest underwent anatomic pulmonary segment resection.The nodule localization time and operating time were significantly different between the two groups(P<0.05). Conclusion In the resection of thoracoscopic lung,subpulmonary,and combined subsegment,IQQA is safe and feasible for the lesion positioning and intraoperative navigation.
