经眉下切口眼睑成形术治疗肥厚型眼睑松弛症的临床效果观察

摘　　要：目的:观察和评价经眉下切口眼睑成形术治疗肥厚型眼睑松弛症的可行性和临床效果。方法:收集2018年1月-2019年12月就诊于河北省眼科医院的双侧肥厚型眼睑松弛症患者共20例,均接受经眉下切口上睑成形术。术中同时处理脱垂的眶部泪腺、增生的副泪腺和膨隆的眶脂肪组织,使上睑恢复正常饱满外观和年轻化。随访时间为术后1周、1个月和3个月。观察指标为眼睑膨隆状态、切口瘢痕淡化程度、上睑皮肤松弛矫正和泪腺复位情况。结果:20例肥厚型眼睑松弛症患者在术后3个月复查时,上睑膨隆消失17例,3例残留轻度膨隆;切口瘢痕完全淡化16例,部分淡化4例;双侧眼睑皮肤松弛情况完全改善10例,明显改善6例,部分改善4例。术后患者均未出现睁眼困难和视物遮挡等症状,均取得满意的治疗和美学效果。结论:眉下切口眼睑成形术对眼睑组织损伤小、手术简便、术后恢复快,是治疗肥厚型眼睑松弛症的理想手术方式。Objective To observe and evaluate the feasibility and clinical effect of blepharoplasty through infra-brow incision for the treatment of hypertrophic blepharoptosis.Methods Twenty patients with bilateral hypertrophic blepharoptosis who were treated in Hebei Provincial Eye Hospital from January 2018 to December 2019 were documented,and all underwent blepharoplasty through infra-eyebrow incision.The prolapsed orbital lacrimal gland,hyperplastic accessory lacrimal gland,and swollen orbital fat tissue were also treated to restore the normal fullness and youthful appearance of the upper eyelid during surgical procedure.The follow-up time was one week,one month and three months postoperatively.Observation indicators include eyelid swelling,incision scar,correction of upper eyelid skin relaxation and lacrimal gland restoration.Results Three months after surgery,17 patients had flat upper eyelid appearance and 3 cases had residual slight bulging were observed in all 20 patients.Additionally,scars in 16 cases were completely faded,and 4 cases were partially faded three months postoperatively.Moreover,10 cases of bilateral eyelid skin relaxation were completely improved,6 cases were significantly improved,and 4 cases were partially improved three months postoperatively.At last,no difficulty of opening eyes and the limitation of visual filed was occurred postoperatively.All patients achieved satisfactory functional and aesthetic effects.Conclusion Blepharoplasty through infra-eyebrow incision has advantages,such as less damage to the eyelid tissue,operate friendly,and quick postoperative recovery.It is an ideal surgical technique for the treatment of hypertrophic blepharoplasty.
