联合筋膜鞘悬吊术与额肌瓣悬吊术治疗重度上睑下垂的美容效果比较

摘　　要：目的:比较联合筋膜鞘悬吊术与额肌瓣悬吊术治疗重度上睑下垂的美容效果。方法:回顾性分析2017年2月-2019年2月在笔者医院进行治疗的78例重度上睑下垂患者一般临床资料,根据治疗方式分组,将采用额肌瓣悬吊术治疗者纳入对照组(n=40),采用联合筋膜鞘悬吊术治疗者纳入观察组(n=38),比较两组患者美容效果以及眼睑恢复情况,记录并发症发生率及满意度。结果:观察组患者上眼睑矫正率显著优于对照组(100.00%vs 80.00%),差异有统计学意义(P<0.05)。观察组治疗后并发症发生率显著低于对照组(5.26%vs 22.50%),差异有统计学意义(P<0.05)。观察组治疗后在上视、平视方面的满意度均明显高于对照组(94.74%vs72.50%,81.58%vs52.50%),差异有统计学意义(P<0.05)。结论:联合筋膜鞘悬吊术治疗重度上睑下垂矫正率显著优于额肌瓣悬吊术,可明显降低术后并发症发生率,患者满意度更高,值得在临床上推广应用。Objective To compare the cosmetic results of conjoint fascial sheath suspension and frontalis muscle flap suspension in the treatment of severe ptosis.Methods The general clinical data of 78 patients with severe ptosis who were treated in the hospital during the period from February 2017 to February 2019 were analyzed retrospectively.According to the treatment method,patients treated with frontalis muscle flap suspension were included into the control group(n=40),and those treated with conjoint fascial sheath suspension were included into the observation group(n=38).The cosmetic results and recovery of eyelids were compared between the two groups.The incidence of complications and satisfaction were recorded.Results The correction rate of upper eyelids in the observation group was significantly better than that in the control group(100.00%vs 80.00%),the difference was statistically significant(P<0.05).The incidence of complications in the observation group after treatment was significantly lower than that in the control group(5.26%vs 22.50%,P<0.05).The satisfaction rates of observation group with looking up and looking at the front horizontally were higher than those of control group(94.74%vs 72.50%,81.58%vs 52.50%,P<0.05).Conclusion The correction rate of severe ptosis by conjoint fascial sheath suspension is significantly superior to that by frontalis muscle flap suspension.The former treatment can significantly reduce the incidence of postoperative complications and patients’satisfaction is relatively higher.
