摘  要:目的探讨光学相干断层扫描血管成像(OCTA)观察冠心病患者视网膜黄斑区血流变化的应用价值。方法采用OCTA观察并比较40例冠心病患者(冠心病组)和35例无心血管疾病的轻度白内障患者(对照组)右眼视网膜中心凹无血管区(FAZ)面积、黄斑区浅层及深层视网膜的血流密度参数。结果冠心病组黄斑区深层视网膜整体、旁中心凹及其颞侧、鼻侧、上方及下方的血流密度均低于对照组(P<0.05)。两组视网膜FAZ面积、黄斑区浅层视网膜血流密度和深层视网膜中心凹血流密度差异均无统计学意义(P>0.05)。结论OCTA检查提示冠心病患者黄斑区深层视网膜整体和旁中心凹血流密度下降,可作为观察冠心病患者微血管变化的指标。Objective To explore the application value of optical coherence tomography angiography(OCTA)in observing the changes of blood flow in macular area of retina in the patients with coronary heart disease(CHD).Methods With OCTA rapid scanning,the area of fovea avascular zone(FAZ),the blood flow density parameters of the macular area in the superficial and deep layers of retina of right eye were observed and compared in 40 CHD patients(group A)and 35 mild cataract patients without cardiovascular disease(group C).Results The blood flow density of whole macular area,para-fovea and its temporal side,nasal side,upper part and lower part in the deep layers of retina in group A were lower than those in group C(P<0.05).There were no significant differences in FAZ area and the blood flow density of the macular area in the superficial layers and the macular fovea in the deep layers of retina between the two groups(P>0.05).Conclusion The OCTA examination can show a decreased blood flow density of whole macular area and para-fovea in the deep layers of retina in CHD patients and can be used as an indicator in observing the microvascular changes in CHD patients.
