温州市婴儿体重生长情况与6月龄贫血状况分析

摘　　要：目的:探讨婴儿从出生到6月龄的体重变化与6月龄贫血发生的相关性。方法:选取温州地区59 786例2016年10月—2017年9月出生的婴儿,参照WHO的0~5岁男、女童年龄别体重表(百分位)将婴儿出生和6月龄的体重分别划为12个生长区间,分析婴儿出生至6月龄的体重生长区间跨度绝对值(以下简称"生长跨度")与6月龄贫血的相关性。结果:婴儿生长跨度越大,6月龄贫血的发生率越高(P<0.001);匹配胎龄、喂养方式等因素后,婴儿4~11个生长跨度和0~3个生长跨度在纯母乳喂养的早产低出生体重儿、混合喂养的早产达标体重儿和纯母乳喂养的足月达标体重儿中6月龄贫血发生率皆存在统计学差异(P<0.05)。结论:体重生长变化虽然不是婴儿6月龄贫血发生的敏感指标,但可初步提醒妇幼人员在日常儿童健康服务中关注对体重生长跨度过大婴儿的血红蛋白监测,必要时给予相应辅助检查及指导。Objective:To explore the correlation between Weight growth from birth to 6 months and the incidence of anemia at 6-month-old.Methods:Selected 59786 infants which Born from October 2016 to September 2017 in Wenzhou,Reference to WHO Weight for age Scale for Boys and Girls aged 0~5(Percentage),then divide the body weight into 12 growth zones when infants were at birth and 6-month-old.Analysis of the relationship between absolute span value of body weight growth interval(Abbreviated as"growth span")from birth to 6 months old and anemia at 6-month-old.Results:Longer the growth span from birth to 6 months old,higher the incidence of anemia at 6-month-old(P<0.001);Matching factors such as gestational age and feeding pattern,the difference between growth span of 0~3 or 4~11 and the incidence of anemia at 6-month-old was statistically significant in Premature low birth weight infants fed exclusively breastfeeding,premature high birth weight infants fed mixed feeding and full-term standard weight infants fed exclusively breastfeeding(P<0.05).Conclusion:Weight span is not a sensitive indicator of anemia in infants aged 6 months,however,the results still preliminarily hints that Staff of Maternal and Child Institutions should Strengthen the surveillance of infants with excessive weight span in daily child health services.
