摘  要:目的:观察高流量湿化氧疗系统对ICU机械通气脱机后患者呼吸状况和气道湿化的影响。方法:选取呼吸衰竭行机械通气脱机后的60例患者为研究对象,按照随机数字表法分为研究组(n=32)和对照组(n=28),对照组采用传统面罩氧气湿化后吸氧,研究组采用高流量湿化氧疗系统将氧气湿化后吸氧,比较两组动脉血pH值、呼吸频率、氧分压(PaO2)、二氧化碳分压(PaCO2)、氧合指数(PaO2/FiO2)水平和气道湿化满意率。结果:氧疗24 h后,两组患者动脉血pH值比较,差异无统计学意义(P>0.05);研究组呼吸频率、PaO2、PaCO2、PaO2/FiO2指数改善情况明显优于对照组,差异有统计学意义(P<0.05);研究组气道湿化满意率高于对照组,差异有统计学意义(P<0.05)。结论:高流量湿化氧疗系统可改善ICU机械通气脱机后患者呼吸状况和提高气道湿化满意率,效果优于传统面罩氧气湿化后吸氧效果。Objective:To explore effects of high-flow humidified oxygen therapy system on respiratory status and airway humidification of patients with off-line ICU mechanical ventilation.Methods:60 patients of respiratory failure after mechanical ventilator weaning were selected as the research objects,and were divided into study group(n=32)and control group(n=28)according to the random number table method.The control group used traditional oxygen mask,while the study group used high-flow humidified oxygen therapy system to humidify the airway and maintain oxygen inhalation.Then,the levels of arterial blood PH,respiratory rate,oxygen partial pressure(PaO2),carbon dioxide partial pressure(PaCO2),oxygenation index(PaO2/FiO2)and airway humidification satisfaction rate were compared between the two groups.Results:24 hours after oxygen therapy,there was no significant difference in the arterial blood pH between the two groups(P>0.05).The improvements of respiration rate,PaO2,PaCO2,and PaO2/FiO2 in the study group were significantly better than those in the control group,and the differences were statistically significant(P<0.05).Further,the satisfaction rate of airway humidification in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusions:High-flow humidified oxygen therapy system for the patients with off-line ICU mechanical ventilation can improve the patients’respiratory statuses.Moreover,its effect on the airway humidification is superior to traditional oxygen mask.
