摘　　要：目的比较8月龄儿童同时接种麻疹风疹联合减毒活疫苗(MR)和乙型脑炎减毒活疫苗(JEV-L)与单独接种MR的免疫原性和不良反应。方法采用随机对照设计,招募8月龄儿童随机分为MR与JEV-L同时接种组(MR+JEV-L组)、MR单独接种组(MR组),检测每名儿童免疫前和免疫后42-48d血清麻疹、风疹IgG抗体,观察免疫后6周内不良反应,分析抗体阳转率、抗体浓度和不良反应发生率。结果MR+JEV-L组、MR组均纳入儿童252名。两组免疫后麻疹抗体阳转率分别为98.81%、99.60%(Fisher确切概率法,P=0.62),IgG抗体浓度中位数分别为1955.36mIU/mL、1885.87mIU/mL(Z=1.02,P=0.24)。两组免疫后风疹抗体阳转率分别为95.63%、96.83%(χ^2=0.49,P=0.48),IgG抗体浓度中位数分别为58.87mIU/mL、62.19mIU/mL(Z=0.80,P=0.54)。两组不良反应发生率分别为11.11%、13.49%(χ^2=0.66,P=0.42),未发生3-4级不良反应。结论8月龄儿童同时接种MR和JEV-L、单独接种MR对麻疹和风疹均具有良好、相当的免疫原性,同时接种未增加不良反应发生率。Objective To compare immunogenicity and adverse reactions f rom simultaneous vaccination with measles and rubella combined attenuated live vaccine(MR)and Japanese encephalitis attenuated live vaccine(JEV-L)versus standalone MR vaccination of 8-month-old infants.Methods We used a randomized,controlled design to recruit 8-month-old infants and allocated them into either a group that received MR and JEV-L simultaneously(MR+JEV-L group)or a group that received MR only(MR group).We tested sera for measles and rubella IgG antibodies before and 42-48 days after vaccination and observed adverse reactions during the 6 weeks following vaccination to determine seroconversion rates,antibody concentrations,and adverse reaction rates.Results We enrolled 252 children into the MR+JEV-L group and into the MR group.Measles seroconversion rates after vaccination were 98.81%and 99.60%,respectively(Fisher’s exact test,P=0.62),with median antibody concentrations of 1955.36 and 1885.87 mIU/mL(Z=1.02,P=0.24).Rubella seroconversion rates were 95.63%and 96.83%,(χ^2=0.49,P=0.48),with median antibody concentrations of 58.87 and 62.19 mIU/mL(Z=0.80,P=0.54).The adverse reaction rate was 11.11%in the MR+JEV-L group and 13.49%in the MR group(χ^2=0.66,P=0.42);no grade 3 or 4 adverse reactions were observed in either group.Conclusions Simultaneous JEV-L and MR administration and standalone MR administration showed good and similar immunogenicity among 8-month-old infants.Simultaneous vaccination did not increase adverse reactions.
