摘　　要：目的观察中医临床适宜技术治疗痔术后疼痛的临床疗效。方法将124例混合痔术后疼痛患者随机分为试验组1(耳穴敷贴组)、试验组2(中药热奄包组)、试验组3(耳穴敷贴+中药热奄包组)及对照组,每组31例,试验组1术晨给予耳穴敷贴治疗;试验组2术后开始每日早晚给予中药热奄包治疗;试验组3术晨给予耳穴敷贴治疗,术后开始每日早晚给予中药热奄包治疗;对照组:仅予基础治疗。各组治疗时间均为5 d,观察治疗前后的临床症状、体征,评定出患者术后、第1、2、5天,肛门疼痛、腹胀、排尿等情况,采用数字评分法,分别对以上观察指标进行评价。结果试验组1显效率51.6%,试验组2显效率48.4%,试验组3显效率64.5%,对照组显效率35.4%,3个试验组与对照组在术后、1 d、2 d、5 d在VAS疼痛评分、尿潴留评分比较均优于对照组(P <0.05);3个试验组之间,试验组3在术后1 d、2 d、5 d的VAS疼痛评分、尿潴留评分的比较上均优于试验组1和试验组2(P <0.05),3个试验组与对照组在腹胀评分对比上无明显差异(P>0.05)。结论联合应用耳穴敷贴及中药热奄包治疗可有效缓解痔术后疼痛及术后尿潴留,值得推广。Objective To observe the clinical effect of appropriate technique of traditional Chinese medicine in treating postoperative pain of hemorrhoids.Methods 124 patients with pain after mixed hemorrhoid operation were randomly divided into experimental group 1(auricular acupoint application group),experimental group 2(Chinese medicine fomentation package group),experimental group 3(auricular acupoint application Chinese medicine fomentation package)and control group,with 31 cases in each group.Experimental group 1 received auricular application treatment in the morning;Experimental group 2 was treated with hot electret bag every morning and evening after operation.Experimental group 3 received auricular application treatment in the morning,and received hot electrowel treatment in the morning and evening after operation.The control group received only basic treatment.The treatment time of each group was 5 days.The clinical symptoms and signs before and after treatment were evaluated,and the patients were evaluated after operation,day 1,2,5,anal pain,abdominal distension,urination and so on.Results The efficiency of experimental group 1 was 51.6%,the efficiency of experimental group 2 was 48.4%,the efficiency of experimental group 3 was 64.5%,the efficiency of control group was 35.4%,the VAS pain score and urinary retention score were better than those of control group(P<0.05).The pain score and urinary retention score were better than those of experimental group 1 and experimental group 2(P<0.05),and there was no significant difference in abdominal distension score between the three experimental groups and the control group(P>0.05).Conclusion The combined application of auricular acupoint application and traditional Chinese medicine fomentation package can effectively relieve the pain after hemorrhoid operation and postoperative urinary retention,which is worth popularizing.
