安神宁心胶囊治疗女性更年期抑郁症的疗效及其对神经、生殖内分泌水平的影响

摘　　要：目的:观察安神宁心胶囊治疗女性更年期抑郁症(female climacteric depression,FCD)的疗效及其对神经、生殖内分泌的影响。方法:将女性更年期抑郁症患者125例随机分为两组,对照组62例口服氟哌噻吨美利曲辛片,观察组63例口服安神宁心胶囊,比较两组患者汉密尔顿抑郁量表(Hamilton depression scale,HAMD)评分、改良Kupperman指数(KMI)、世界卫生组织生存质量测定量表简表(World Health Organization Quality of life,WHOQOL-BREF)评分和血清5-羟色胺(5-hydroxy tryptamine,5-HT)、去甲肾上腺素(norepinephrine,NE)、多巴胺(dopamine,DA)3种单胺类神经递质水平及雌二醇(estradiol,E2)、促卵泡激素(follicle stimulating hormone,FSH)、黄体生成素(luteinizing hormone,LH)3种生殖相关激素水平,并比较两组临床疗效。结果:治疗后两组患者HAMD评分、改良Kupperman指数降低(P<0.05),WHOQOL-BREF评分升高(P<0.05);DA、NE、5-HT、E2水平升高(P<0.05),FSH和LH水平降低(P<0.05)。各指标改善观察组均优于对照组,总有效率观察组为85.7%,与对照组的85.5%比较差异有统计学意义(P>0.05)。结论:安神宁心胶囊治疗FCD与氟哌噻吨美利曲辛片疗效相当,其药理机制可能与改善神经、生殖内分泌水平有关。Objective: To observe clinical effects of Anshen Ningxin capsules in the treatment of FCD and its influence on neuroendocrine and reproductive endocrine. Methods: All 125 patients were randomly allocated to two groups, 62 cases of the control group took flupentixol and melitracen tablets orally, and 63 cases of the observation group Anshen Ningxin capsules, to compare HAMD scores, KMI, WHOQOL-BREF scores and the levels of three kinds of monoamine neurotransmitters including 5-HT, NE and DA, and three kinds of reproductive hormones including E2, FSH and LH. Results: After treating, HAMD scores and KMI scores of both groups lowered(P<0.05),WHOQOL-BREF scores lifted(P<0.05);the levels of DA, NE, 5-HT and E2 rose, the levels of FSH and LH reduced.The observation group was better than the control group in the improvements of different indexes, total effective rate of the observation group was 85.7%, higher than 85.5% of the control group, the difference had statistrcal meaning(P>0.05). Conclusion: Anshen Ningxin capsulesisequivalentto flupentixoland melitracen tabletsin treatingFCD, and its pharmacological mechanism might be related to improving the levels of neuroendocrine and reproductive endocrine.
