试论李东垣对孙一奎辨治臂痛思想的影响

摘　　要：通过阅读、学习李东垣和孙一奎的相关医著,并结合《内经》《伤寒论》的学术思想,从“外感内伤,分论臂痛”“三阴三阳,分论臂痛”两方面总结李东垣对孙一奎辨治臂痛思想的影响。李东垣治病尊仲景之旨,将“外感内伤”“三阴三阳辨证”理论应用于臂痛的治疗之中,为臂痛的临证分型论治开拓了思路,广泛影响了孙一奎等后世医家。孙一奎在辨病之时常引东垣之述,尤其在《赤水玄珠·臂痛》中主张从外感内伤、三阴三阳分论臂痛,确立了明确的理法方药,增加了臂痛论治方法的完整性。Through reading and studying the related medical works of LI Dongyuan and SUN Yikui,and combining the academic thoughts of Internal Classic and Treatise on Cold Damage Diseases,this paper summarizes the influence of LI Dongyuan on SUN Yikui’s thought of treating brachialgia from two aspects of“To differentiate external contraction from internal injury,and then treats brachialgia respectively”and“To differentiate three-yin from three-yang,and then treats brachialgia respectively”.LI Dongyuan respected Zhongjing’s opinion of treating the disease,applied the theory of“To differentiate external contraction from internal injury”and“three-yin and threeyang syndrome differentiation”to the treatment of brachialgia,opened up the thinking for the clinical syndrome typing and differential treatment of it,and widely influenced the doctors of later generations,namely SUN Yikui etc.SUN Yikui often cited Dongyuan’s statement in the disease differentiation,especially in Chishui Xuanzhu:brachialgia,which advocated treating brachialgia respectively by differentiation external contraction from internal injury and differentiation three-yin from three-yang,established a definite system of theory,method,prescription and medicines,and increased the integrity of brachialgia differential treatment method.
