高频彩色超声在腹壁子宫内膜异位症中的诊断价值分析

摘　　要：目的探讨高频彩色超声在腹壁子宫内膜异位症(AWE)中的效果和应用价值。方法对确诊的120例AWE患者进行术前高频彩色超声筛查,并与术后病理检查结果作对比,分析不同病灶大小的超声声像特征,计算超声诊断阳性率和内异症分期诊断符合率。结果大小病灶组间的潜伏期、病灶直径、形态、病灶深度、强回声及CDFI血流等指标比较差异有统计学意义(P<0.05);超声诊断阳性率为95.00%(114/120),子宫内膜异位症诊断分期符合率为87.50%(105/120)。结论高频彩色超声不仅可以发现AWE的病灶,而且还能够确定病变累及范围,具有较高的诊断阳性率和分期诊断符合率,可作为AWE的首选影像学检查。Objective To explore value and application value of high frequency color ultrasound diagnosing abdominal wall endometriosis( AWE). Methods 120 cases definite AWE patients were screened by high frequency color ultrasound before operation,and compared with pathological examination results after operation. Ultrasound sonographic characteristics were compared between different lesion sizes,and positive rate of ultrasonic diagnosis and diagnostic coincidence rate of staging of endometriosis were computed. Results The indexes of incubation period,lesion diameter,focus form,depth of lesion,strong echo and CDFI blood flow existed statistical significance between the small lesion group and the big lesion group( P<0. 05). Positive rate of ultrasonic diagnosis was 95. 00%( 114/120),and diagnostic coincidence rate of staging of endometriosis was 87. 50%( 105/120). Conclusion High frequency color doppler could not only discover AWE lesions,but also determine the extent of involvement of lesions. It has a high positive rate of ultrasonic diagnosis and diagnostic coincidence rate of staging of endometriosis. So it can be considered as the first choice of imaging examination for AWE.
