栀黄解毒饮联合磷酸奥司他韦治疗儿童甲型流感的疗效及对炎症因子的影响  

Effect of Using Zhihuang Jiedu Decoction Combined with Oseltamivir Phosphate in the Treatment of Influenza A in Children and Its Influnce on Inflammatory Factors

摘　　要：目的:观察中药栀黄解毒饮联合磷酸奥司他韦治疗儿童甲型流感的临床疗效。方法:将130例甲型流感患儿按照随机数字表法分为2组,每组65例,对照组应用磷酸奥司他韦口服,药物剂量根据体重而定。治疗组在对照组治疗基础上应用中药栀黄解毒饮口服,每日1剂。两组均连续治疗7d。比较两组主要症状发热、咳嗽、咽痛消失时间,并比较两组治疗前后中医证候评分、血清白细胞介素-4(IL-4)、干扰素-γ(IFN-γ)水平和IFN-γ/IL-4比值。并比较两组治疗总有效率。结果:治疗组主要症状发热、咳嗽、咽痛消失时间少于对照组(P<0.01);治疗后中医证候评分低于对照组(P<0.01);血清IL-4高于对照组,IFN-γ水平及IFN-γ/IL-4值均低于对照组(P<0.05,P<0.01);治疗总有效率高于对照组(P<0.05)。差异均有统计学意义。结论:栀黄解毒饮联合磷酸奥司他韦可有效改善症状、缓解病情,治疗儿童甲型流感效果显著,可能与通过某种机制节血清IL-4、IFN-γ水平,逆转Th1/Th2细胞失衡有关。Objective:To investigate the clinical efficacy of Zhihuang Jiedu Decoction combined with Oseltamivir Phosphate on influenza A in children.Methods:130 patients with influenza A were divided into two groups according to the digital random method,with 65 in each group.The controlled group were given Oseltamivir Phosphate orally,drug dosage depends on body weight.On the basis of the controlled group,the treatment group were treated with Zhihuang Jiedu Decoction orally,one dose a day.Both groups were treated continuously for 7 days.The disappearance time of fever,cough and pharyngalgia in the two groups were compared,and the TCM syndrome scores,serum interleukin-4(IL-4),interferon-gamma(IFN-gamma)levels and IFN-gamma/IL-4 ratio were compared before and after treatment.The total effective rate was compared between the two groups.Results:The main symptoms of fever,cough and pharyngalgia disappeared less in the treatment group than in the controlled group(P<0.01);The scores of TCM syndrome after treatment were lower than those of the controlled group(P<0.01);The serum IL-4 level was higher than that of the controlled group,and the levels of IFN-gamma and IFN-gamma/IL-4 were lower than those of the controlled group(P<0.05,P<0.01).The differences were statistically significant.Conclusion:Zhihuang Jiedu Decoction combined with Oseltamivir Phosphate can effectively improve symptoms and alleviate illness,and it has a significant effect on influenza A in Children.It may be related to reversing the imbalance of Th1/Th2 cells by regulating serum levels of IL-4 and IFN-gamma through some mechanism.
