针药联用法治疗脑卒中后遗症肩手综合征作用机制的研究

摘　　要：目的研究针药联用法治疗脑卒中后遗症肩手综合征的潜在作用机制。方法选取2016年2月-2017年10月河北省邯郸市中医院脑二科收治的脑卒中肩手综合征患者90例为研究对象,按照随机数表法分为观察组与对照组,每组45例。对照组采用康复治疗;观察组在康复治疗基础上予以针药联用法进行治疗。治疗3周后比较分析两组患者治疗前后血脂和血液流变学指标的变化。白细胞介素2(IL-2)、白细胞介素6(IL-6)、白细胞介素10(IL-10)和肿瘤细胞坏死因子α(TNF-α)水平。结果与同组治疗前比较,两组患者治疗后血脂中总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)水平,血流变学各项指标,炎症因子中IL-2、IL-6、TNF-α水平均降低,高密度脂蛋白(HDL-C)和IL-10水平均升高,差异有统计学意义(P<0.05)。与对照组治疗后比较,观察组治疗后血脂中总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)水平,血流变学各项指标,炎症因子IL-2、IL-6、TNF-α水平均降低,高密度脂蛋白(HDL-C)和IL-10水平升高,差异有统计学意义(P<0.05)。结论针药联用法用于脑卒中肩手后遗症患者的治疗,能够有效抑制炎性反应,促进血液循环,有效改善患者肩手综合征症状。Objective To investigate the acupuncture combined with medicine therapy in the treatment of shoulder-hand syndrome of stroke sequelae.MethodsA total of 90 cases of stroke patients with shoulder-hand syndrome admitted to the second Department of Brain of Handan Hospital of Traditional Chinese Medicine from February 2016 to October 2017 were selected as the study objects,and they were randomly divided into observation group and control group,with 45 cases in each group.The control group was treated with rehabilitation therapy;while the observation group was treated with acupuncture combined with medicine therapy on the basis of rehabilitation treatment.After 3 weeks of the treatments,the changes of blood lipid and hemorheology were compared between the two groups before and after the treatments.The levels of IL-2,IL-6,IL-10 and TNF-αwere measured.ResultsCompared with that before the treatment in the same group,the levels of total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL-C),hemorheology indexes,IL-2,IL-6 and TNF-αin inflammatory factors in the two groups were decreased,while the levels of high-density lipoprotein(HDL-C)and IL-10 were increased(P<0.05).Compared with the control group after the treatment,the levels of TC,TG,LDL-C,hemorheology indexes,IL-2,IL-6 and TNF-αin the observation group were decreased,while the levels of HDL-C and IL-10 were increased(P<0.05).ConclusionThe application of acupuncture combined with medicine therapy can effectively reduce inflammation,promote blood circulation and improve the symptoms of shoulder hand syndrome after stroke.
