二胎妊娠产后抑郁风险与家庭支持影响因素的分析

摘　　要：目的探讨二胎妊娠产后抑郁风险与家庭关怀影响因素。方法选取2018年4-8月在某三甲医院建立孕期检查档案的350名二胎妊娠晚期孕妇,采用一般资料调查表、产后抑郁量表(C-PDSS)、家庭关怀度指数测评表对350名二胎妊娠晚期孕妇进行问卷调查。结果350名二胎妊娠晚期孕妇C-PDSS平均得分为(52.28±4.43)分,88名(25.14%)发生产后抑郁风险。家庭关怀度评分与C-PDSS得分之间存在负相关,差异有统计学意义(P<0.05)。二胎妊娠晚期孕妇C-PDSS的影响因素分析结果显示,是否担心胎儿健康,产后照护,对胎儿性别的期望,是否担心分娩的疼痛,孕期睡眠质量等因素差异有统计学意义(P均<0.05)。结论对二胎妊娠孕产妇产后抑郁风险应充分关注,虽然孕产妇有过生育经验,但对再次妊娠的心理、生理、家庭支持问题,仍需进行有效的筛查和早期干预,利于保障二胎孕妇顺利度过妊娠期。Objective To explore the risk of postpartum depression in second pregnancy and the influencing factors of family care.Methods350 pregnant women in the third trimester of their second pregnancy were investigated by general data questionnaire,postpartum depression scale(C-PDSS)and family care index.ResultsThe average score of C-PDSS of 350 second trimester pregnant women was(52.28±4.43),and 88(25.14%)were at risk of postpartum depression.There was a negative correlation between score of family care and C-PDSS,the difference was statistically significant(P<0.05).The analysis of the influencing factors of C-PDSS in the second trimester of pregnancy showed that there were statistically significant differences(P<0.05).ConclusionWe should pay full attention to the risk of postpartum depression in pregnant women with second child.Although pregnant women have experience in childbirth,effective screening and early intervention are still needed for psychological,physical and family support problems of second pregnancy,so as to ensure that pregnant women with second child can successfully pass the pregnancy period.
