摘　　要：原发性痛经是妇女常见疾病之一,是指盆腔和生殖器无明显病变的前提下在行经期间发生的腹部疼痛。对于原发性痛经的发病机制各方医家仍有争论,但痛经本身从中医学角度出发,是由气血不通和气血不荣所导致的冲任失调而致疼痛。目前临床上痛经患者多以实证为主,其证型主要为气滞血瘀、湿热瘀阻、寒凝血瘀,虚证者以气血虚弱为主。现代医学实验证明,原发性痛经与下列因素有关:血管加压素、雌激素、遗传因素、神经及神经递质、催产素、前列腺素、β-内啡肽、免疫、精神等。调查发现,越来越多的年轻女性患有原发性痛经,并且呈现递增趋势,严重干扰女性身心健康,因此对原发性痛经的治疗备受瞩目。目前现代医学对原发性痛经的治疗首选药物多为口服避孕药、非甾体类抗炎药、维生素、钙离子通道阻滞剂等,因其见效快,被广泛使用,但通过对调查发现大量使用上述药物,不良反应是显而易见的,尤其以胃肠反应、水钠潴留最为常见,中医治疗原发性痛经开始受到重视。中医治疗优势如下:中医对痛经采用辨证治疗的方法,具有靶向治疗作用,而且其特点是可以联合推拿、艾灸、敷脐等疗法,是使效率大幅增高,且无不良反应。综上,中医治疗原发性痛经疗效显著,无不良反应,通过如下几种方法对原发性痛经的治疗进行论述:中药汤剂治疗、中成药治疗、中医外治法等方法,并探讨其在原发性痛经的治疗优势。Primary dysmenorrhea is one of the common diseases of Chinese women,which refers to the abdominal pain during menstruation under the premise of no obvious lesions of pelvic cavity and genitalia. The pathogenesis of primary dysmenorrhea is still debated among doctors,but dysmenorrhea itself from the perspective of traditional Chinese medicine can be considered to be caused by Qi and blood obstruction and Qi and blood loss of nourishing resulting in pain. At present,clinical dysmenorrhea patients are mainly excessive syndrome,and the syndrome types are mainly Qi stagnation and blood stasis,damp-heat and blood stasis and cold-coagulation and blood stasis. And the deficiency syndrome is mainly deficiency of Qi and blood. A large number of modern medical experiments have proved that primary dysphoria is related to the following factors: vasopressin,estrogen,genetic factors,nerves and neurotransmitters,oxytocin,prostaglandins,endorphins,immunityand spirit. According to the survey,more and more young female patients suffer from primary dysmenorrhea,and the trend is increasing,which seriously interferes with the physical and mental health of women,and so the treatment of primary dysmenorrhea has attracted much attention. At present,the first choice drugs for the treatment of primary dysmenorrhea in modern medicine are oral contraceptives,non-steroidal anti-inflammatory drugs,vitamins,calcium channel blockers and so on.Due to its quick effect,it is widely used. But through the investigation of patients,it is found that a large dose use of the drugs can lead to obviousadverse reactions,especially gastrointestinal reactions and water and sodium retention is the most common,and so the treatment of primary dysmenorrhea begins to pay attention to traditional Chinese medicine.
