温脾养胃汤治疗慢性胃炎脾胃虚寒证临床疗效

摘　　要：目的探讨温脾养胃汤治疗慢性胃炎脾胃虚寒证的临床疗效。方法选择2017年10月-2019年4月收治的80例慢性胃炎脾胃虚寒证患者,随机分为两组,各40例。对照组采用西药口服治疗,在此基础上,观察组采用温脾养胃汤治疗,对比两组临床疗效、临床症状、胃肠黏膜屏障功能、胃功能、血清炎性因子、不良反应发生率。结果观察组总有效率较对照组高(P<0.05)。治疗前,两组临床症状、胃肠黏膜屏障功能指标、胃功能指标、血清炎性因子水平比较,差异无统计学意义(P>0.05),治疗后,观察组胃黏膜充血、水肿、糜烂和胆汁反流发生率较对照组低,观察组二胺氧化酶(diamine oxidase,DAO)、肠脂肪酸结合蛋白(intestinal fatty acid binding protein,IFABP)、D-乳酸水平较对照组低,观察组胃蛋白酶原Ⅰ(pepsinogenⅠ,PGⅠ)、胃蛋白酶原Ⅱ(pepsinogenⅡ,PGⅡ)、胃泌素-17(gastrin-17,G-17)水平较对照组高,观察组肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(Interleukin-6,IL-6)、C反应蛋白(C-reactive protein,CRP)水平较对照组低(P<0.05)。观察组不良反应发生率较对照组高,差异无统计学意义(P>0.05)。结论温脾养胃汤治疗慢性胃炎脾胃虚寒证临床疗效显著,可减轻临床症状,改善胃肠黏膜屏障功能及胃功能,降低血清炎性因子表达水平,且安全性较高。Objective To investigate the clinical effect of Wenpi Yangwei decoction in the treatment of chronic gastritis with deficiency cold in spleen and stomach syndrome.Methods From October 2017 to April 2019,80 patients with chronic gastritis with deficiency cold in spleen and stomach syndrome were randomly divided into two groups(n=40).On the basis of oral treatment with western medicine,the observation group was treated with Wenpi Yangwei decoction.The clinical efficacy,clinical symptoms,gastrointestinal barrier function,gastric function,serum inflammatory factors and the incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).Before treatment,the two groups’clinical symptoms,the functional index of the gastrointestinal mucosa barrier,the function index of the stomach and the levels of the serum inflammatory factors were not significant(P>0.05).After treatment,the incidence of hyperemia,edema and erosion and bile reflux in the observation group were lower than those of the control group,and the diamine oxidase(DAO)in the group was observed.The levels of intestine fatty acid binding protein(IFABP),D-lactic acid,pepsinogenⅠ(PGⅠ),pepsinogen II(PGⅡ)and gastrin-17(G-17)were higher in the observation group than those in the control group.The level of C-reactive protein(CRP)in the observation group was lower than that in the control group(P<0.05).The incidence of adverse reaction in the observation group was higher than that of the control group,but the difference was not significant(P>0.05).Conclusion The The curative effect of Wenpi Yangwei decoction on the treatment of chronic gastritis with deficiency cold in spleen and stomach syndrome is significant.It can relieve the clinical symptoms,improve the function of the gastrointestinal mucosa barrier and the function of the stomach,decrease the levels of the serum inflammatory factors,and the safety is high.
