无创助产干预促进自然分娩及降低会阴侧切率的效果  

Effect Evaluation of Noninvasive Midwifery Intervention in Promoting Natural Delivery and Reducing Perineal Lateral Incision Rate

摘　　要：目的探讨无创助产干预促进自然分娩及降低会阴侧切率的效果。方法选取2017年7月至2018年7月丹东市妇女儿童医院收治的280例产妇,随机将其分为研究组和对照组,各140例。研究组采取无创助产干预,对照组采取传统助产技术。比较两组产妇会阴裂伤程度、总产程时间、自然分娩率、顺转剖率及产后2 h出血量。结果研究组无裂伤占37.14%,Ⅰ度裂伤占36.42%,Ⅱ度裂伤占26.42%,无Ⅲ、Ⅳ度裂伤,对照组无裂伤占25.71%,Ⅰ度裂伤占30.00%,Ⅱ度裂伤占31.43%,Ⅲ度裂伤占12.86%,无Ⅳ度裂伤,差异均有统计学意义(均P<0.05)。研究组总产程时间为(48.37±11.62)min,自然分娩119例(85.00%),顺转剖21例(25.00%),会阴侧切51例(36.43%),产后2 h出血量为(194.24±36.36)mL;对照组总产程时间为(41.26±14.24)min,自然分娩113例(80.71%),顺转剖48例(29.29%),会阴侧切107例(76.43%),产后2 h出血量为(203.46±43.92)mL。两组自然分娩率、总产程时间、产后2 h出血量比较,差异均无统计学意义(均P>0.05);但会阴侧切率比较,差异有统计学意义(P<0.05)。结论无创助产干预是保护软产道、降低会阴侧切率的有效途径,且不会降低自然分娩率。Objective To investigate the effect evaluation of noninvasive midwifery intervention in promoting natural delivery and reducing perineal lateral incision rate.Methods From July 2017 to July 2018,280 cases of puerpera were randomly divided into two groups,140 cases in each group.The research group adopted non-invasive midwifery intervention,while the control group adopted traditional midwifery technique.The degree of perineal laceration,total duration of labor,natural delivery rate,antepartum incision rate and postpartum blood loss were compared between the two groups.Results In the research group,no laceration accounted for 37.14%,Ⅰdegree laceration accounted for 36.42%,Ⅱdegree laceration accounted for 26.42%,withoutⅢ,Ⅳdegree laceration occurred,while in the control group,no laceration accounted for 25.71%,Ⅰdegree laceration accounted for 30.00%,Ⅱdegree laceration accounted for 31.43%,Ⅲdegree laceration accounted for 12.86%,withoutⅣdegree laceration occurred,and the differences were statistically significant(all P<0.05).In the study group,the total duration of labor in the study group was(48.37±11.62)min,and there were 119 cases(85.00%)of natural childbirth,21 cases(25.00%)of retrograde incision,51 cases(36.43%)of lateral perineal incision,and postpartum blood loss at 2 hours was(194.24±36.36)mL.In the control group,the total duration of labor in the study group was(41.26±14.24)min,and there were 113 cases(80.71%)of natural childbirth,48 cases(29.29%)of retrograde incision,207 cases(76.43%)of lateral perineal incision,and postpartum blood loss at 2 hours was(203.46±43.92)mL.The natural delivery rate,total labor duration,and postpartum blood loss were not statistically significant between the two groups(all P>0.05),but the perineal lateral resection rate were statistically significant(P<0.05).Conclusion Noninvasive midwifery intervention is an effective way to protect the soft birth canal and reduce the rate of perineal lateral incision without reducing the rate of spontaneous delivery.
