硬膜外麻醉复合全身麻醉用于直肠癌根治手术的麻醉效果

摘　　要：目的探讨硬膜外麻醉复合全身麻醉应用于直肠癌根治手术中的麻醉效果。方法于我院2016年10月至2018年10月期间接受直肠癌根治手术治疗的患者中以抽签方法选出150例参与本次研究,以患者的身体状况以及应用的麻醉手段为依据分为对照组和观察组,每组75例。对照组患者在手术过程中实施常规全身麻醉,观察组患者在手术过程中实施硬膜外麻醉复合全身麻醉。比较两组患者采用不同麻醉手段的麻醉效果、麻醉30 min后的麻醉生理指标以及不良反应发生情况。结果观察组患者的麻醉优良率98.67%高于对照组患者的81.33%(P<0.05)。观察组患者麻醉30 min后的平均动脉压(74.82±4.12)mm Hg和心率(73.13±7.52)次/分均优于对照组患者的(86.33±5.25)mm Hg、(65.35±6.70)次/分(P<0.05)。观察组患者不良反应发生率5.33%%低于对照组患者的21.33%(P<0.05)。结论硬膜外麻醉复合全身麻醉应用于直肠癌根治术患者,不仅具有麻醉效果更加理想的优势,而且可进一步确保患者手术过程中麻醉生理指标的稳定性,并切实降低患者不良反应的发生率。Objective To study the effect of epidural anesthesia combined with general anesthesia in radical operation of rectal cancer.Methods from October 2016 to October 2018,150 patients undergoing radical operation for rectal cancer were selected by drawing lots to participate in this study.The patients were divided into control group and observation group based on their physical condition and applied anesthesia methods,with 75 cases in each group.Patients in the control group received conventional general anesthesia during the operation,and patients in the observation group received epidural anesthesia combined with general anesthesia during the operation.The effects of anesthesia,the physiological indicators of anesthesia 30 minutes after anesthesia,and the occurrence of adverse reactions were compared between the two groups of patients using different anesthesia methods.Results The excellent and good rate of anesthesia in the observation group was 98.67%,which was higher than 81.33%in the control group(P<0.05).The average arterial pressure and heart rate after 30 minutes of anesthesia of the observation group were(74.82±4.12)mm Hg and(73.13±7.52)beats/min,which were better than(86.33±5.25)mm Hg and(65.35±6.70)beats/min.The in the control group(P<0.05).The incidence of adverse reactions in the observation group was 5.33%,which was lower than 21.33%in the control group(P<0.05).Conclusion The method of epidural anesthesia combined with general anesthesia in the treatment of patients with radical rectal cancer not only has the ideal advantages of anesthetic effect,but also can further ensure the stability of the anesthesia physiological indexs during the operation,and effectively reduce the incidence of adverse reactions.
