扩创植皮术联合负压封闭引流技术对大面积烧伤感染难愈创面患者的治疗效果

摘　　要：目的分析研究扩创植皮术联合负压封闭引流技术(VSD)对大面积烧伤感染难愈创面患者的治疗效果。方法选择我院在2014年3月至2018年7月期间收治的70例大面积烧伤感染难愈创面患者作为研究对象,按照硬币法将患者分为对照组和观察组,每组35例。对照组患者采用扩创植皮术治疗,观察组患者采用扩创植皮术联合VSD治疗。比较两组患者术后的创面愈合率、皮片成活率、创面愈合时间、并发症发生率。结果观察组患者的创面愈合率、皮片成活率、创面愈合时间分别为91.43%、82.86%、(16.92±5.01)d,均明显优于对照组的37.14%、40.00%、(27.47±4.82)d,差异具有统计学意义(P<0.05)。观察组患者的并发症发生率5.71%低于对照组患者的22.86%,差异具有统计学意义(P<0.05)。结论扩创植皮术联合VSD治疗大面积烧伤感染难愈创面患者具有确切的治疗效果,且并发症发生率低。Objective To analyze and study the treatment effect of expansion and skin grafting combined with vacuum sealing drainage(VSD)on refractory and infected wounds in large area burns.Methods The 70 patients with infected and refractory wounds in large-area burns admitted in our hospital between March 2014 and July 2018 were selected as the research objects.According to the coin method,the patients were divided into a control group and an observation group,with 35 cases in each group.Patients in the control group were treated with expanded skin grafting,and patients in the observation group were treated with expanded skin grafting combined with VSD.The postoperative wound healing rate,skin survival rate,wound healing time,and complication rate of the two groups were compared.Results The wound healing rate,skin survival rate,and wound healing time of the observation group were 91.43%,82.86%,and(16.92±5.01)d,respectively,which were significantly better than 37.14%,40.00%,and(27.47±4.82)d of the control group,the difference was statistically significant(P<0.05).The complication rate of the observation group was 5.71%,which was lower than the 22.86%of the control group,the difference was statistically significant(P<0.05).Conclusion Expansion and skin grafting combined with VSD in the treatment of on patients with infected and refractory wounds in large area burns has a definite therapeutic effect,and the incidence of complications is low.
