肋骨骨折实施多层螺旋CT三维重建与DR胸片诊断的价值

摘　　要：目的对比多层螺旋CT三维重建、DR胸片在肋骨骨折诊断方面的应用价值。方法收集我院在2015年3月至2019年1月收治的90例疑似肋骨骨折患者,经患者同意后分别实施多层螺旋CT三维重建与DR胸片诊断,并跟访到病理诊断结果。评价两种方法对肋骨骨折的诊断价值。结果①90例疑似肋骨骨折患者经病理诊断后76例为肋骨骨折,14例为非肋骨骨折;②对比单发性肋骨骨折、多发性肋骨骨折、隐匿性肋骨骨折检出率,多层螺旋CT检出率均高于DR胸片检查结果(P<0.05);③评价不同方法诊断准确率、特异性以及敏感度,多层螺旋CT检查上述结果显著高于DR胸片检查结果(P<0.05)。结论多层螺旋CT三维重建与DR胸片在肋骨骨折诊断方面均有着重要价值,相对而言,多层螺旋CT三维重建对肋骨骨折的检出率更高,其诊断准确率、特异性、灵敏度更具有优势,在条件允许的情况下可作为首选诊断方法。Objective To compare the application value of multi-slice spiral CT three-dimensional reconstruction and DR chest radiograph in the diagnosis of rib fractures.Methods Collected 90 patients with suspected rib fractures admitted to our hospital from March 2015 to January 2019.After the patient's consent,they performed multi-slice spiral CT three-dimensional reconstruction and DR chest radiograph diagnosis,and followed up the pathological diagnosis results.To evaluate the diagnostic value of the two methods for rib fractures.Results(1)After 90 patients with suspected rib fractures were pathologically diagnosed,76 cases were rib fractures and 14 cases were non-rib fractures.(2)Compared the detection rate of single rib fracture,multiple rib fractures,and occult rib fractures,detected by multi-slice spiral CT The rates were higher than the results of DR chest radiographs(P<0.05).(3)To evaluate the diagnostic accuracy,specificity and sensitivity of different methods,the above results of multi-slice spiral CT were significantly higher than the results of DR chest radiographs(P<0.05).Conclusions Multi-slice spiral CT three-dimensional reconstruction and DR chest radiograph are both of great value in the diagnosis of rib fractures.Relatively speaking,multi-slice spiral CT three-dimensional reconstruction has a higher detection rate for rib fractures,and its diagnostic accuracy,specificity,and sensitivity are higher.It has advantages and can be used as the preferred diagnostic method when conditions permit.
