摘 要:目的:探讨多标记后延迟时间(PLD)三维动脉自旋标记(3D-ASL)成像技术在评估不同程度脑白质疏松(LA)患者脑血流灌注改变方面的应用价值。方法:回顾性收集2017年1月至2020年3月于脑病科及康复科收治LA患者80例,男46例,女34例,年龄42~80岁,平均(60.6±9.4)岁。均采用GE 3.0T超导磁共振行常规序列及3D-ASL成像检查,依据T2 Flair序列图像,采用Fazekas量表综合评估LA患者脑室周围和脑实质深部病灶,并进行三级分组。分别测量轻、中、重三组患者脑室旁白质区脑血流量(CBF),同时统计达峰CBF值及达峰时间。结果:轻、中、重三组患者脑室旁白质区达峰CBF值差异无统计学差异(P>0.05),但随着LA程度加重,CBF值达峰时间发生延迟。结论:不同程度LA患者脑白质区CBF峰值无明显差异,但随着LA程度加重,脑实质血液流动速度减慢。Objective To explore the application value of 3d-asl in assessing the changes of cerebral blood flow perfusion in patients with different degrees of leukoaraiosis(LA).Methods From January 2017 to March 2020,80 cases of La patients,46 males and 34 females,aged 42~80 years,with an average age of(60.6±9.4)years,were collected retrospectively.All patients were examined by conventional sequence and 3d-asl imaging with Ge 3.0T superconductive MRI.According to T2 FLAIR sequence image,Fazekas scale was used to comprehensively evaluate the deep lesions around the ventricle and brain parenchyma of La patients,and three-level grouping was carried out.Cerebral blood flow(CBF)in the paraventricular white matter area was measured in the light,medium and heavy groups,and the peak CBF value and time were counted.Results There was no significant difference in CBF between the light,medium and heavy groups(P>0.05),but with the aggravation of La,the time of CBF reaching the peak was delayed.Conclusion There is no significant difference in CBF peak in white matter area of patients with different degrees of La,but with the aggravation of La,the blood flow speed of brain parenchyma slows down.
