摘 要:目的探讨闭环式血糖智能住院-社区-家庭管理平台对2型糖尿病患者的应用效果,为医护人员提供一种有效控制患者血糖的管理模式。方法采用非随机对照试验法,选择本院内分泌二病区110例2型糖尿病患者作为对照组,内分泌一区110例2型糖尿病患者作为试验组,对照组住院及出院后采用常规糖尿病管理,试验组采用闭环式血糖智能管理平台进行住院-社区-家庭管理。干预1年后,比较两组患者各项临床代谢指标(空腹血糖、餐后2h血糖、糖化血红蛋白、甘油三酯、总胆固醇、BMI)及自我管理能力各维度情况。结果两组患者干预后各项临床代谢指标及自我管理能力各维度情况比较,均P<0.05,差异具有统计学意义,试验组各项指标优于对照组,饮食控制、遵医用药、血糖达标合格率均高于对照组。结论闭环式血糖智能住院-社区-家庭管理平台的应用,能改善2型糖尿病患者血糖、糖化血红蛋白、血脂、体质指数,提高患者自我管理能力,逐步实现住院-社区-家庭信息无缝对接的2型糖尿病患者管理模式。Objective To explore the effect of closed-loop hospital-community-family intelligent blood glucose management platform on blood glucose control of type 2 diabetic patients.Methods Non-randomized controlled trial and cluster random sampling method were used to select 110 patients with type 2 diabetes from Ward 2 of the endocrinology department of our hospital as the control group and 110 patients with type 2 diabetes from Ward 1 as the trial group.In the former group routine diabetes management was done in hospital and after discharge,and in the latter the closed-loop hospital-community-family intelligent blood glucose management platform was used.After 12 months of intervention,the two groups were compared in terms of fasting blood glucose,2-hour postprandial blood glucose,glycosylated hemoglobin,triglyceride,serum total cholesterol,body mass index and diabetes self-management ability.Results The trial group was significantly better than the control group in terms of clinical metabolic indexes as well as all the items of self-management(P<0.05).The pass rates of the trial group were significantly higher than those of the control group in terms of diet control,medication compliance and blood glucose control.Conclusions The closed-loop hospital-community-family intelligent blood glucose management platform is effective in improving not only the levels of blood glucose,glycosylated hemoglobin,blood lipid and body mass index of patients with type 2 diabetes, but also the patients' self-management ability. It helps with the full-course management across hospital, community and family.
