常规超声及非血管超声造影在腹部含液病变置管引流中的应用

摘　　要：目的评估常规超声及NV CEUS在腹部含液病变经皮穿刺置管引流术中的应用价值。方法我院腹部感染性含液病变拟进行经皮穿刺置管引流的326例患者,术前明确病变部位、大小、数目、空间关系;术中明确引流管前端的位置;术后明确病变是否彻底引流及引流是否通畅。结果326例患者术前含液性病变体积最大值2643930 mm^3,最小值1046.67 mm^3,中位数133986.4 mm^3;置管引流后3 d和/或7 d复查体积最大值792771 mm^3,最小值0 mm^3,中位数4892.64 mm^3。326例患者引流后,93例病灶完全消失,224例病灶明显缩小,2例病灶范围增大,7例数据缺失,有效率99.37%(317/319)。本研究病例术后未见出血、周围器官损伤等并发症。结论NV CEUS可持续强化引流目标,在一定程度上增强了穿刺的准确度,并在评估疗效及适时拔除引流管方面具有指导意义。Objective To evaluate the application value of conventional ultrasound and NV CEUS in percutaneous drainage of abdominal fluid collection.Methods 326 cases of abdominal infectious fluid collection in our hospital were planned to be treated by percutaneous drainage.The location,size,number and spatial relationship of each lesion was determined before the procedure,and the position of each front end of the drainage tube was determined during the procedure,and whether the fluid collections were completely drained and whether the drainage was unobstructed were determined after the procedure.Results The volume of fluid collections with the 326 patients had a maximum of 2643930 mm^3,a minimum of 1046.67 mm^3,and a median of 133986.4 mm^3 before the procedure;The maximum volume of 792771 mm^3,a minimum volume of 0 mm^3,and a median volume of 4892.64 mm^3 were reexamined the 3 rd and/or 7 th days after the drainage.After drainage in 326 patients,93 lesions were completely disappeared,224 lesions were significantly reduced,2 lesions were enlarged,7 patients′data were missing.The effective rate was 99.37%(317/319).There were no complications such as bleeding and injury of peripheral organs.Conclusions NV CEUS can enhance the accuracy of puncture to a certain extent,and has guiding significance in the evaluation of curative effect and timely removal of drainage tube.
