摘　　要：目的探讨输卵管妊娠破裂和卵巢黄体囊肿破裂的CT特征及诊断价值。方法回顾性分析2016年1月至2019年7月南宁市第二人民医院收治的41例经手术病理证实的输卵管妊娠破裂和卵巢黄体囊肿破裂患者的临床及CT资料,分析两者的CT表现特征及鉴别诊断要点。结果41例均表现为腹盆腔积血,以子宫及附件周围明显,而输卵管妊娠破裂的出血量通常较多。输卵管妊娠破裂特征性表现为一侧宫旁或附件区混杂密度包块,周围积血包绕,包块中见圆形或卵圆形低密度孕囊;黄体囊肿破裂特征性表现为一侧宫旁或附件区不规整的囊状低密度影,周围积血或伴血肿;黄体囊肿的径线大多较孕囊的径线大,两者比较差异有统计学意义(P<0.05)。结论输卵管妊娠破裂和卵巢黄体囊肿破裂的CT表现均具有一定的特征,术前CT扫描可为临床提供较准确的诊断信息。Objective To investigate the features and diagnostic value of computed tomography(CT)for ruptured tubal ectopic pregnancy and corpus luteum cyst rupture.Methods The clinical and CT data from 41 patients with ruptured tubal ectopic pregnancy or corpus luteum cyst rupture confirmed by operation and pathology in the Second People’s Hospital of Nanning from January 2016 to July 2019 were retrospectively analyzed.The CT features and differential diagnosis points of ruptured tubal ectopic pregnancy and corpus luteum cyst rupture were analyzed.Results CT results of the 41 cases manifested abdominal and pelvic hemorrhage,which was obvious around the uterus and adnexa,while hemorrhage volume of ruptured tubal ectopic pregnancy was usually large.Ruptured tubal ectopic pregnancy was characterized by a mixed density mass on one side of the uterine or adnexal area,surrounded by blood,with round or oval low-density gestational sac in the mass according to CT.Ovarian corpus luteum cyst rupture was characterized by irregular saccular low-density shadows on one side of the uterine or adnexal area,with hematocele around or hematoma.The diameters of most of corpus luteum cysts were larger than those of the gestational sacs,and the difference was statistically significant(P<0.05).Conclusions CT manifestations of ruptured tubal ectopic pregnancy and corpus luteum cyst rupture have certain characteristics.Preoperative CT scan can provide more accurate diagnostic information in clinical.
