胃内镜下黏膜剥离术后联合应用康复新液与奥美拉唑的效果观察

摘　　要：目的探讨胃内镜下黏膜剥离术(ESD)术后患者联合应用康复新液与奥美拉唑的临床效果。方法选取2018年10月至2019年12月濮阳市人民医院收治的需行胃ESD患者68例,根据随机数字表法将其分为对照组和观察组,每组34例。观察组术后使用奥美拉唑联合康复新口服液,对照组术后仅应用奥美拉唑,两组均持续治疗8周。比较两组术后症状、溃疡面积、血清血管内皮生长因子(VEGF)水平及出血情况。结果术后第3、7天,观察组腹胀、返酸、嗳气及恶心等临床症状评分低于对照组(P<0.05)。术后第4、8周,观察组溃疡面积小于对照组(P<0.05)。术后第8周,观察组VEGF水平高于对照组(P<0.05)。两组术后迟发性出血情况比较差异未见统计学意义(P>0.05)。结论康复新液联合奥美拉唑可有效改善患者胃ESD术后腹痛、腹胀、返酸、嗳气等临床症状,缩小溃疡面积,并提升机体VEGF水平,有助于促进患者术后康复,但对出血症状改善不明显,临床治疗需选择性使用。Objective To investigate the effects of Kangfuxin liquid combined with omeprazole on patients after gastric endoscopic submucosal dissection(ESD).Methods Sixty-eight patients underwent gastric ESD in Puyang People’s Hospital from October 2018 to December 2019 were enrolled,and they were divided into two groups by random number table method,with 34 cases in each group.After operation,control group received omeprazole,while observation group received Kangfuxin liquid combined with omeprazole.And the subsequent treatment lasted for 8 weeks.Then related indexes were compared between the two groups,including postoperative symptoms,ulcer area,serum vascular endothelial growth factor(VEGF)level and bleeding.Results On the 3rd and 7th days after operation,the scores of abdominal distention,acid reflux,belching and nausea in the observation group were lower than those in the control group(P<0.05).The ulcer area of the observation group was smaller than that of the control group at the 4th and 8th weeks after operation(P<0.05).The VEGF level in the observation group was higher than that in the control group in the 8th week after operation(P<0.05).There was no significant difference between the two groups in bleeding status(P>0.05).Conclusions Application of Kangfuxin liquid combined with omeprazole after gastric ESD can effectively ameliorate the clinical symptoms,such as abdominal distension,acid reflux,belching,and nausea,shrink the ulcer area,improve VEGF level and promote the recovery;however,its hemostatic efficacy is not obvious,so the clinical treatment should be based on the patients’conditions.
