摘  要:目的研究重组人生长激素(r-hGH)治疗特发性矮小(ISS)儿童的效果及对糖脂代谢、甲状腺功能的影响。方法选择郑州儿童医院2014年1月至2018年10月诊治的79例ISS患儿作为研究对象,根据其治疗方式分为对照组(25例)、研究1组(26例)和研究2组(28例),对照组给予基础支持治疗,研究1、2组在对照组治疗基础上分别给予小剂量、大剂量r-hGH治疗。三组均治疗1年,比较三组治疗前后糖脂代谢水平及甲状腺功能,并评估疗效和安全性。结果治疗前,三组身高、生长速率和骨龄比较差异未见统计学意义(P>0.05);治疗后,研究1、2组患儿上述指标升高且高于对照组,且研究2组高于研究1组(P<0.05)。治疗后,三组患儿糖脂代谢指标、甲状腺功能指标水平较治疗前有波动幅度,但是差异未见统计学意义(P>0.05)。三组患者不良反应发生率比较差异未见统计学意义(P>0.05)。结论r-hGH应用于ISS患儿的治疗具有明显的促生长作用,且大剂量效果更佳,同时对其糖脂代谢及甲状腺功能影响较小。Objective To study the curative effects of recombinant human growth hormone(r-hGH)on idiopathic short stature(ISS),and its influence on glucose and lipid metabolism and thyroid function.Methods A total of 79 children with ISS diagnosed and treated in Zhengzhou Children’s Hospital from January 2014 to October 2018 were selected as the research subjects.And they were divided into the control group(25 cases),study group 1(26 cases)and study group 2(28 cases)according to the treatment methods.The control group was given basic supportive treatment,and the two study groups were respectively treated with low dose and high dose of r-hGH based on the treatment for control group.All of the three groups were treated for 1 year.The glucose and lipid metabolism levels and thyroid function in the three groups were compared before and after treatment.The curative effect and safety were evaluated.Results There was no statistically significant difference in height,growth velocity or bone age among the three groups before treatment(P>0.05).After treatment,the above indicators were improved in the three groups,and they were significantly larger in the two study groups than those in the control group,moreover,they were higher in the study group 2 than in the study group 1(P<0.05).There were slight changes in levels of glucose and lipid metabolism indexes and thyroid function indexes in the three groups before and after treatment,but the difference was not significant(P>0.05).There were no statistically significant differences in incidence of adverse reactions among the three groups(P>0.05).Conclusions R-hGH has obvious growth-promoting effects in the treatment of children with ISS,and the effect of high-dose r-hGH is better,with little effect on glucose and lipid metabolism and thyroid function.
