合并肾功能不全的急性冠状动脉综合征患者行经皮冠状动脉介入治疗的有效性及安全性评价

摘　　要：目的:探讨急性冠状动脉综合征合并肾功能不全患者行皮冠状动脉介入(PCI)治疗的效果及其安全性。方法:抽取2017年6月至2019年2月山西省人民医院收治的急性冠状动脉综合征合并肾功能不全患者56例作为观察对象,分别采用PCI(观察组,130例)和药物保守方案(对照组,126例)进行治疗,观察分析两组患者的治疗效果及其心血管不良事件的发生情况。结果:两组患者经治疗后肌酐水平均有一定变化,差异未见统计学意义(P>0.05),组间治疗前、治疗后肌酐水平比较,差异未见统计学意义(P>0.05);观察组患者在治疗后1年其心绞痛发作率、靶血管重建率、心力衰竭发作率均低于对照组,差异有统计学意义(P<0.05)。结论:对合并肾功能不全的急性冠状动脉综合征患者实施经皮冠状动脉介入术治疗,可有效降低此类患者心血管不良事件的发生风险,对于其远期预后的改善具有十分积极的作用。Objective To investigate the effect and safety of percutaneous coronary intervention(PCI)in patients with acute coronary syndrome and renal insufficiency.Methods A total of 56 patients with acute coronary syndrome and renal insufficiency treated in Shanxi People’s Hospital from June 2017 to February 2019 were selected as observation objects,and were treated with PCI(observation group,30 cases)and drug conservative regimen(control group,26 cases).The therapeutic effect and the occurrence of cardiovascular adverse events were observed and analyzed.Results After treatment,the creatinine level of the two groups changed to some extent,there was no statistical difference(P>0.05),and there was no significant difference between the two groups before and after treatment(P>0.05);within 1 year after treatment,the incidence of angina pectoris,target vessel reconstruction rate and heart failure in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusions Percutaneous coronary intervention for patients with acute coronary syndrome with renal insufficiency can effectively reduce the risk of cardiovascular adverse events in such patients and has a very positive effect on improving their long-term prognosis.
