摘 要:目的:观察补阳还五汤联合硫辛酸对糖尿病周围神经病变患者体内C肽水平和抗氧化能力的影响。方法:选取糖尿病周围神经病变患者104例,按随机数字表法分为对照组和观察组各52例。对照组给予基础治疗联合硫辛酸治疗,观察组在基础治疗上给予补阳还五汤联合硫辛酸治疗。检测2组神经传导速度、氧化应激指标、C肽和糖化血红蛋白(HbA1c)水平,观察不良反应发生情况,统计临床疗效。结果:观察组总有效率为90.38%,高于对照组73.08%,差异有统计学意义(P<0.05)。治疗后,2组正中神经和腓总神经的运动神经传导速度(MNCV)、感觉神经传导速度(SNCV)水平均较治疗前上升(P<0.05),观察组正中神经和腓总神经的MNCV、SNCV水平高于对照组(P<0.05)。治疗后,2组HbA1c水平较治疗前下降,观察组空腹C肽(FC-P)、餐后2 h C肽(2hC-P)水平较治疗前升高,差异均有统计学意义(P<0.05)。治疗后,观察组2hC-P水平高于对照组,HbA1c水平低于对照组,差异均有统计学意义(P<0.05)。治疗后,2组总抗氧化能力(T-AOC)、超氧化物歧化酶(SOD)水平较治疗前升高,丙二醛(MDA)水平较治疗前下降,差异有统计学意义(P<0.05)。治疗后,观察组T-AOC、SOD水平高于对照组,MDA水平低于对照组(P<0.05)。2组不良反应发生情况比较,差异无统计学意义(χ2=0.210,P=0.247)。结论:补阳还五汤联合硫辛酸治疗糖尿病周围神经病变疗效显著,可调节餐后2 h血清C肽和糖化血红蛋白水平,提高机体总抗氧化能力,减轻氧化应激损伤,修复受损神经,且安全性高。Objective:To observe the effect of Buyang Huanwu tang combined with lipoic acid on the level of C-peptide and antioxidant capacity in patients with diabetic peripheral neuropathy. Methods: A total of 104 cases of patients with diabetic peripheral neuropathy were divided into the control group and the observation group according to the random number table method, 52 cases in each group. The control group was given basic treatment combined with lipoic acid, and the observation group was additionally treated with Buyang Huanwu tang based on the treatment of the control group. The nerve conduction velocity,oxidative stress parameters,and the levels of C-peptide and hemoglobin A1 c(Hb A1 c) in the two groups were detected. The incidence of adverse reactions was observed. The clinical effect was counted. Results:The total effective rate was 90.38% in the observation group,higher than that of 73.08% in the control group,the difference being significant(P<0.05). After treatment, the levels of motor nerve conduction velocity(MNCV) and sensory nerve conduction velocity(SNCV) of median nerve and general peroneal nerve in the two groups were increased when compared with those before treatment(P<0.05),and the above levels in the observation group were higher than those in the control group(P<0.05). After treatment,the levels of Hb A1 c in the two groups were increased when compared with those before treatment,and the levels of fasting C-peptide(FC-P) and postprandial 2 h C-peptide(2 h C-P) in the observation group were increased when compared with those before treatment,differences being significant(P<0.05). After treatment,the level of 2 h C-P in the observation group was higher than that in the control group,and the level of Hb A1 c was lower than that in the control group,differences being significant(P<0.05).
