补阳还五汤联合常规西药治疗老年带状疱疹后遗神经痛临床研究  

Clinical Study on Buyang Huanwu Tang Combined with Routine Western Medicine for Post-Herpetic Neuralgia in Senile Patients

摘　　要：目的:观察补阳还五汤联合常规西药治疗老年带状疱疹后遗神经痛(PHN)的临床效果。方法:选取76例确诊为PHN的老年患者,按随机数字表法分为对照组和观察组各38例。对照组采用维生素B1片、甲钴胺片及加巴喷丁胶囊治疗,观察组在此基础上给予补阳还五汤治疗。治疗15 d后,比较2组生活质量评分(QLS)、视觉模拟评分法(VAS)评分、血清T淋巴细胞亚群、临床疗效及不良反应情况。结果:治疗后,2组QLS、VAS评分均降低,观察组QLS、VAS评分低于对照组(P<0.05);2组血清T淋巴细胞亚群CD3+、CD4+、CD4+/CD8+水平均上升,观察组CD3+、CD4+、CD4+/CD8+水平高于对照组(P<0.05);2组CD8+水平均下降,观察组CD8+水平低于对照组(P<0.05)。观察组总有效率为89.47%,高于对照组的65.79%(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:补阳还五汤联合常规西药治疗老年PHN患者,可提高其免疫功能,疗效佳,安全性高。Objective:To observe the clinical effect of Buyang Huanwu tang combined with routine western medicine for post-herpetic neuralgia(PHN) in senile patients. Methods:A total of 76 cases of senile patients diagnosed with PHN were selected and divided into the control group and the observation group according to the random number table method,38 cases in each group. The control group was treated with vitamin B1 tablets,mecobalamin tablets and gabapentin capsules,and the observation group was additionally given Buyang Huanwu tang based on the treatment of the control group. After 15 days of treatment, Quality of Life Score(QLS) and the scores of Visual Analogue Scale(VAS), T lymphocyte subsets in serum,clinical effect and adverse reactions in the two groups were compared. Results:After treatment,the scores of QLS and VAS in the two groups were decreased,and the scores in the observation group were lower than those in the control group(P<0.05);the levels of T lymphocyte subsets including CD3+,CD4+and CD4+/CD8+in serum in the two groups were increased,and the levels in the observation group were higher than those in the control group(P<0.05);the levels of CD8+in the two groups were decreased,and the level in the observation group was lower than that in the control group(P<0.05). The total effective rate was 89.47% in the observation group,higher than that of 65.79% in the control group(P<0.05). There was no significant difference in the comparison of adverse reactions between the two groups(P>0.05). Conclusion:The therapy of Buyang Huanwu tang combined with routine western medicine for PHN in senile patients can improve the immune function,with high safety and good curative effect.
