头针结合康复训练治疗痉挛型脑瘫临床研究

摘　　要：目的:观察头针结合康复训练治疗对痉挛型脑瘫患儿粗大运动功能测试量表(GMFM-88)评分、改良Ashworth痉挛及综合功能评分的影响。方法:将本院80例痉挛型脑瘫患儿按随机数字表法分为对照组和观察组各40例。对照组给予康复训练治疗,观察组在对照组治疗方案的基础上给予头针治疗,比较2组疗效,治疗前后彩色多普勒超声检查结果 [大脑中动脉(MCA)收缩期峰值流速(Vs)、平均血流速度(Vm)和血管阻力指数(RI)]、下肢关节活动度[髋关节(大腿内收肌角)、膝关节(腘窝角)和踝关节(足背屈角)]、粗大运动功能测试量表(GMFM-88)评分、改良Ashworth痉挛分级量表(MAS)及综合功能评分。结果:治疗后,观察组Vs、Vm、大腿内收肌角、腘窝角大于对照组(P<0.05),观察组RI与对照组比较,差异无统计学意义(P>0.05),足背屈角小于对照组(P<0.05)。治疗后,观察组GMFM-88评分、综合功能评分均高于对照组(P<0.05),MAS评分低于对照组(P<0.05)。临床疗效总有效率观察组92.50%,高于对照组67.50%(P<0.05)。结论:头针结合康复训练治疗可提高治疗效果,改善痉挛型脑瘫患儿GMFM-88评分、MAS及综合功能评分,促进运动功能恢复。Objective:To observe the effect of scalp acupuncture combined with rehabilitation training on the scores of Gross Motor Function Measure-88(GMFM-88), Modified Ashworth Scale and comprehensive function in children with spastic cerebral palsy. Methods:A total of 80 cases of children with spastic cerebral palsy in our hospital were divided into the control group and the observation group according to the random number table method,40 cases in each group. The control group was given rehabilitation training, and the observation group was additionally given scalp acupuncture based on the treatment of the control group. The curative effect, the results of color Doppler ultrasonography including middle cerebral artery(MCA),systolic peak velocity(Vs),mean velocity(Vm) and vascular resistance index(RI),the range of motion of lower limbs including adductor angles of the thigh in the hip joint, popliteal fossa angles in the knee joint and angles of foot dorsiflexion in the ankle joint,as well as the scores of Gross Motor Function Measure-88(GMFM-88),Modified Ashworth rating-Scale(MAS) and comprehensive function were compared between the two groups. Results: After treatment, Vs,Vm,the adductor angle of the thigh,as well as the popliteal fossa angle in the observation group were lager than those in the control group(P<0.05);there was no significant difference being found in the comparison of RI between the two groups(P>0.05);the angle of foot dorsiflexion in the observation group was smaller than that in the control group(P<0.05). After treatment,the scores of GMFM-88 and comprehensive function in the observation group were both higher than those in the control group(P<0.05), and the score of MAS was lower than that in the control group(P<0.05). The effective rate of treatment was 92.50% in the observation group,higher than that of 72.50 in the control group(P<0.05).
