血清IL-17和IL-6及TNF-α与儿童特应性体质肺炎支原体感染的相关性

摘　　要：目的探讨血清白细胞介素-17(IL-17)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)与儿童特应性体质肺炎支原体肺部感染之间的关系。方法选取2017年1月-2019年7月于苏州第九人民医院就诊的肺炎支原体肺部感染患儿共80例,根据其是否为特应性体质分为研究组和对照组,研究组为特应性体质患儿,共32例,对照组为正常体质患儿,共48例。采用酶联免疫吸附法对IL-17、IL-6、TNF-α及常规指标进行检测;采用免疫分析系统对患儿特应性体质进行检测。结果研究组患儿接受糖皮质激素治疗、吸氧治疗的比例高于对照组(P<0.05);有特应性皮炎史和哮喘史的患儿例数,研究组高于对照组(P<0.05)。研究组患儿血清IL-17表达水平低于对照组(P<0.05);研究组IL-6、TNF-α表达水平高于对照组(P<0.05)。研究组患儿血清总IgE高于对照组(P<0.05)。联合检测的诊断效能优于单个指标检测。结论血清IL-17水平较低,IL-6、TNF-α水平较高,免疫球蛋白含量较高的特应性体质患儿患肺炎支原体肺部感染的病情较为严重。血清IL-17、IL-6、TNF-α水平可用于诊断特应性体质儿童肺炎支原体肺部感染。OBJECTIVE To explore the correlation between the serum interleukin-17(IL-17),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and the Mycoplasma pneumoniae pulmonary infection in children with atopic constitution.METHODS A total of 80 children with M.pneumoniae pulmonary infection who were treated in the Ninth People’s Hospital of Suzhou from Jan 2017 to Jul 2019 were enrolled in the study and divided into the study group and the control group according to the status of atopic constitution,32 children with atopic constitution were assigned as the study group,and 48 children with normal constitution were set as the control group.The IL-17,IL-6,TNF-αand routine indexes were detected by using enzyme-linked immunosorbent assay,and the atopic constitution of the children was examined with the use of immunoassay system.RESULTS The proportion of the children who were treated with glucocorticoids and oxygen therapy was significantly higher in the study group than in the control group(P<0.05).The number of the children who had the history of atopic dermatitis and asthma was significantly larger in the study group than in the control group(P<0.05).The serum IL-17 level of the study group was significantly lower than that of the control group(P<0.05);the levels of IL-6 and TNF-αof the study group were significantly higher than those of the control group(P<0.05).The serum total IgE level of the study group was significantly higher than that of the control group(P<0.05).The diagnostic efficiency of the joint detection of the indexes was significantly higher than that of the detection of single index(P<0.05).CONCLUSION The M.pneumoniae pulmonary infection is more severe in the atopic constitution children who have low level of serum IL-17,high levels of IL-6,TNF-αand immunoglobulins.The serum IL-17,IL-6 and TNF-αcan be used for diagnosis of M.pneumoniae pulmonary infection in the atopic constitution children.
