摘 要:目的:分析早期神经移植移位术治疗新生儿臂丛损伤的疗效。方法:选取2017年1月至2019年12月平原县第一人民医院新生儿科收治的72例臂丛神经损伤新生儿,所有患儿均采取早期神经移植和移位手术。比较患儿手术前、后肩肘关节与手功能评分的变化。结果:72例患儿中术前肩关节功能评定优10例、良22例、差40例,术后肩关节功能评定优36例、良30例、差6例,患儿手术前、后肩关节功能评定结果差异有统计学意义(χ^2=5.632,P=0.046)。72例患儿中术前肘关节功能评定1分23例、2分11例、3分15例、4分12例、5分11例,术后肘关节功能评定1分8例、2分12例、3分12例、4分18例、5分22例,患儿手术前、后肘关节功能评定结果差异有统计学意义(χ^2=5.523,P=0.045)。72例患儿中术前手功能评定1分20例、2分15例、3分16例、4分15例、5分6例,术后手功能评定1分7例、2分10例、3分11例、4分20例、5分24例,患儿手术前、后手功能评定结果差异有统计学意义(χ^2=5.413,P=0.044)。结论:早期神经移植移位术应用于新生儿臂丛神经损伤有助于改善患儿肩、肘、手等部位的功能状况,有助于提高患儿的上肢功能,值得推广使用。Objective:To analyze the effect of early nerve transplantation for brachial plexus injury in neonates.Methods:From January 2017 to December 2019,72 cases of neonates with brachial plexus injury were hospitalized.All the children underwent early nerve transplantation and displacement surgery to compare the changes of shoulder and elbow joint and hand function scores before and after surgery.Results:In this 72 cases of preoperative shoulder function evaluation,10 cases were excellent,22 cases were good,and 40 cases were poor.After the operation,shoulder function evaluation was found to be excellent in 36 cases,good in 30 cases,and poor in 6 cases.The difference in shoulder joint function evaluation results was statistically significant(χ^2=5.632,P=0.046).In this preoperative evaluation of 72 patients with elbow function,there were 23 cases with 1 point,11 cases with 2 points,15 cases with 3 points,12 cases with 4 points,and 11 cases with 5 points.After the operation,the elbow joint function evaluation showed that there were 8 cases with 1 point,12 cases with 2 points,12 cases with 3 points,18 cases with 4 points,and 22 cases with 5 points(χ^2=5.523,P=0.045).In the 72 patients with preoperative hand function evaluation,there were 20 cases with 1 point,15 cases with 21 points,16 cases with 3 points,15 cases with 4 points,and 6 cases with 5 points.Postoperative hand function evaluation found that there were 7 cases in 1 point,10 cases in 2 point,11 cases in 3 point,20 cases in 4 points,and 24 cases in 5 points.The difference in hand function evaluation before and after the operation was statistically significant(χ^2=5.413,P=0.044).Conclusions:The application of early nerve transplantation in the treatment of neonatal brachial plexus injury can help improve the functional status of the children's shoulders,elbows,hands and other parts,and improve the children's upper extremity function.It is worthy of popularization.
