25羟维生素D3、血尿酸水平与2型糖尿病患者周围神经病变的关系

摘　　要：目的分析血清中25羟维生素D3(25-OH-D3)及血尿酸(serum uric acid,SUA)与2型糖尿病(type 2 diabetes,T2DM)患者周围神经病变(diabetic peripheral neuropathy,DPN)的关系。方法选取2型糖尿病患者396例为研究对象,根据糖尿病周围神经病变的有无分为:正常组,184例,异常组,212例。比较两组患者的年龄、糖尿病病程、身体质量指数(BMI)、收缩压(SBP)、舒张压(DBP)、空腹血糖(FPG)、甘油三酯(TG)、胆固醇(TC)、25-OH-D3、低密度脂蛋白(LDL-C)、血清C肽、促甲状腺激素(TSH)、尿微量白蛋白/肌酐(ACR)、糖化血红蛋白(HbA1c)、血尿酸(SUA)情况。根据数字震动感觉阈值检查仪SensiometerA通用的诊断标准将异常组分为轻中度风险组(双脚VPT≤25,112例)、重度风险组(双脚VPT>25,100例),分别比较两组年龄、病程、HbA1c、TC、LDL-C、TG、血清C肽、25-OH-D3、SUA。结果正常组SUA水平低于异常组,差异具有统计学意义(P<0.05);异常组25-OH-D3水平低于正常组,差异具有统计学意义(P<0.05)。异常组HbA1c、TG、糖尿病病程、TC、平均年龄、收缩压、FPG水平高于正常组,差异具有统计学意义(P<0.05)。结论糖尿病周围神经病变患者25-OH-D3及SUA水平与周围神经病变关系密切,且25-OH-D3是糖尿病周围神经病的保护性因素,SUA是DPN的危险性因素。Objective To analyze the relationship between 25-hydroxyvitamin D3(25-OH-D3),Serum uric acid and peripheral neuropathy in patients with type 2 diabetes mellitus.Methods 396 patients with type 2 diabetes were selected and divided into the normal group(184 cases)and the abnormal group(212 cases)according to the presence or absence of diabetic peripheral neuropathy.The two groups were compared in terms of age,duration of diabetes,body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),fasting blood glucose(FPG),triglyceride(TG),cholesterol(TC),25-OH-D3,low density lipoprotein(LDL-C),serum C peptide,thyroid stimulating hormone(TSH),urine microalbumin/creatinine(ACR),glycated hemoglobin(HbA1c)and serum uric acid(SUA).According to the general diagnostic criteria of Sensiometer A,a digital vibration sensory threshold tester,the abnormal components were divided into light and moderate risk groups(both feet VPT≤25,112 cases)and severe risk groups(both feet VPT>25,100 cases).Age,course of disease,HbA1c,TC,LDL-C,TG,serum C peptide,25-OH-D3,SUA were compared respectively between the two groups.Results The SUA level in the normal group was lower than that in the abnormal group,and the difference was statistically significant(P<0.05);the 25-OH-D3 level in the abnormal group was lower than that in the normal group,and the difference was statistically significant(P<0.05).The abnormal group had higher HbA1c,TG,duration of diabetes,TC,mean age,systolic blood pressure,and FPG levels than the normal group,and the difference was statistically significant(P<0.05).Conclusion The levels of 25-OH-D3 and SUA in patients with diabetic peripheral neuropathy are closely related to peripheral neuropathy.Besides,25-OH-D3 is a protective factor for diabetic peripheral neuropathy,and SUA is a risk factor for DPN.
