天麻素注射液治疗后循环缺血性眩晕有效性及安全性Meta分析

摘　　要：目的系统评价天麻素注射液治疗后循环缺血性眩晕(PCIV)的有效性与安全性。方法利用计算机对PubMed、Embase、Cochrane Library、CBM、CNKI、万方、维普等数据库进行检索,收集关于天麻素注射液治疗PCIV的RCTs,检索时限为建库至2020年2月7日。采用改良Jadad量表对纳入研究的RCTs进行方法学质量评价,并使用Cochrane协作网提供的RevMan5.3软件进行数据分析。结果共纳入11项RCTs,累计受试者932例。Meta分析结果显示,与血栓通/血塞通注射液+基础治疗相比,天麻素注射液+血栓通/血塞通注射液+基础治疗PCIV的疗效更优[OR=5.44,95%CI(2.87,10.30),Z=5.20,P<0.00001];与基础治疗相比,天麻素注射液单用或联合基础治疗PCIV的疗效更优[OR=6.12,95%CI(2.62,14.32),Z=4.18,P<0.0001];与复方丹参注射液相比,天麻素注射液治疗PCIV的疗效更优[OR=11.46,95%CI(3.32,39.56),Z=3.86,P=0.0001]。结论天麻素注射液辅助治疗PCIV具有较好的疗效,安全性方面仍需更多高质量研究予以证实。Objective To systematically evaluate the effectiveness and safety of gastrodin injection after treatment of circulatory ischemic vertigo(PCIV).Methods A computer was used to search PubMed,Embase,Cochrane Library,CBM,CNKI,Wanfang,Weipu and other databases to collect RCTs on the treatment of PCIV with gastrodin injection.The search time limit is from the establishment of the database to February 7,2020.The modified Jadad scale was used to evaluate the methodological quality of the included RCTs,and the RevMan5.3 software provided by the Cochrane Collaboration was used for data analysis.Results A total of 11 RCTs were included,with a total of 932 subjects.Meta-analysis results show that compared with Xueshuantong/Xuesaitong injection+basic treatment,gastrodin injection+Xueshuantong/Xuesaitong injection+basic treatment of PCIV is more effective[OR=5.44,95%CI(2.87,10.30),Z=5.20,P<0.00001];Compared with basic treatment,gastrodin injection alone or combined with basic treatment of PCIV is more effective[OR=6.12,95%CI(2.62,14.32),Z=4.18,P<0.0001];Compared with compound Danshen injection,gastrodin injection is more effective in treating PCIV[OR=11.46,95%CI(3.32,39.56),Z=3.86,P=0.0001].Conclusion Gastrodin injection has a good effect in adjuvant treatment of PCIV,and more high-quality studies are needed to confirm its safety.
