摘 要:目的从基因水平了解湖北地区艾滋病病毒(HIV)感染组与AIDS患者组的人类白细胞抗原HLA等位基因频率,比较两类人群的差异,初步探索此人群HLA等位基因与AIDS疾病进程的相关性。方法采用PCR-SSOP及PCR-SBT进行HLA基因的基因分型。使用Arlequin ver3.0软件分析2组HLA-A、B、DRB 1等位基因、Hardy-Weinberg平衡检测及单体型的频率,使用Epi Info 7软件和SPSS18.0软件分析病毒感染组与AIDS患者组的HLA等位基因差异。结果 Logistic回归分析显示B~*57∶01对疾病进程可能有保护作用,而DRB1~*04∶05可能是AIDS患病的危险因素,并独立于宿主年龄、性别和感染方式的影响作用。另外,有些常见HLA单体型的个体可能预示着对HIV感染疾病进程有影响。纯合HLA-A、B、DRB1等位基因及HLA-Bw4/Bw6基因对AIDS的疾病进程无影响。结论此研究推证了人类自身遗传因素中HLA基因多态性影响AIDS疾病进程。Objective To investigated the association between HLA alleles, haplotypes and AIDS progression in HIV-1 infected patients by analyzing and comparing the differences gene frequencies of HLA alleles(A, B, DRB1) and haplotypes in HIV-1 infected and AIDS individuals in Hubei province of China. Methods HLA-A, B, and DRB1 allele typing was performed using polymerase chain reaction sequence specific oligonucleotide(PCR-SSOP) and polymerase chain reaction-sequencing based typing(PCR-SBT) assays. HLA-A, B, DRB1 alleles, Hardy-Weinberg equilibrium and haplotype frequencies of the two groups were analyzed using Arlequin ver3.0 software. The differences of HLA alleles between the two groups were analyzed by Epi Info 7 software and SPSS18.0 software.Results The results of Logistic regression analysis revealed that B~*57∶01 seemed resistant to AIDS progression, and the presence of DRB1~*04∶05 was associated with a poor disease outcome in HIV-1 infection. These associations were independent of age, sex, and transmission route of the host. In addition, some common HLA haplotypes may indicate an impact on the progression of HIV infection. No association was observed between HLA-A, B, DRB1 homozygotes, HLA-Bw4, Bw6 and AIDS progression. Conclusion HLA gene polymorphism has a significant role in HIV-1 infection/AIDS progression.
