妊娠合并胎盘血管瘤6例临床分析

摘　　要：目的探讨胎盘血管瘤的临床表现、诊断及孕期管理。方法回顾性分析昆山市第一人民医院2013~2019年收治的6例胎盘血管瘤的临床资料。结果4例胎盘血管瘤直径超过5 cm,2例出现羊水污染Ⅱ度,3例出现子宫收缩乏力(其中1例产后出血),1例直径10 cm巨大胎盘血管瘤母胎出现严重并发症,胎死宫内,母体出现妊娠期高血压,引产过程并发胎盘早剥,羊水栓塞,危及产妇生命,1例出现胎膜早破;2例直径<5 cm中1例孕期无明显并发症,1例出现羊水污染Ⅰ度。结论对于胎盘血管瘤,临床不应忽视,可能危及母胎生命。特别是妊娠中期发现胎盘血管瘤,应加强产检次数,密切随访瘤体生长速度、羊水量、胎儿生长情况及母体并发症。Objective To explore the clinical manifestations,diagnosis and management of pregnancy during placental hemangioma.Methods The clinical data of 6 cases of placental hemangioma admitted to Kunshan First People's Hospital from 2013 to 2019 were retrospectively analyzed.Results 4 cases of placental hemangioma exceeded 5 cm in diameter,2 cases had amniotic fluid contamination ofⅡdegree,3 cases of uterine contraction weakness(including 1 postpartum hemorrhage),1 case of 10 cm diameter placental hemangioma had serious complications and fetal death in utero,maternal hypertension occurred during pregnancy,induced placental abruption,amniotic fluid embolism,and endangered maternal life.1 case had premature rupture of membranes;2 cases<5 cm in diameter,1 case had no obvious complications during pregnancy,and 1 case occurred amniotic fluid pollutionⅠdegree.Conclusion For placental hemangioma,it should not be ignored clinically,it may endanger maternal and fetal life.Especially when placental hemangioma is found in the second trimester,the number of birth tests should be strengthened,and the tumor growth rate,amniotic fluid volume,fetal growth and maternal complications should be closely followed.
