益气健脾法对老年肠内营养性腹泻的临床研究

摘　　要：[目的]探讨中医"益气健脾法"在老年肠内营养性腹泻患者中治疗作用,探索相关的机制,为防治此类疾病提供思路。[方法]27例符合老年肠内营养性腹泻患者随机分为中药组和西药组,同时选取行肠内营养治疗无腹泻患者作为"正常对照"研究,测量治疗前后肠道屏障功能指标"内毒素、二胺氧化酶、人肠脂肪酸结合蛋白"的变化。[结果]肠内营养性腹泻患者中药组治疗后血清内毒素水平明显下降,二胺氧化酶(DAO)、人肠脂肪酸结合蛋白(I-FABP)水平经过治疗后有下降,但差异无统计学意义。[结论]研究结果显示中药益气健脾法可以有效治疗肠内营养性腹泻、减轻症状、改善肠道功能,机制可能与改善肠道屏障功能有关。西药益生菌治疗具有一定疗效和安全性。[Objective]To observe the therapeutical effect of tonifying Qi and spleen method in care of elderly patients with enteral nutrition-related diarrhea and to explore the possible mechanism of traditional Chinese medicine.[Methods]27 elderly patients who needed enteral nutrition with diarrhea were chosen and randomly divided into experimental group who take the traditional Chinese medicine(n=15) and control group with probiotics(n=12).Meanwhile 12 patients who needed enteral nutrition without diarrhea were chosen as study subjects.The plasma DAO and I-FABP activity and Enotoxin in the sera were detected before and after the treatment.[Results]The plasma enotoxin was significantly lower(P<0.05) in the experimental group.The plasma DAO and I-FABP activity were declined,but not significant.At the same time,indexes of safety were not significantly changed.[Conclusion]The efficacy of tonifying Qi and spleen method in care of enteral nutrition-related diarrhea is remarkable.Protective effects on the gut mucosa barrier was the probable reason.Probiotics also have a certain effect on diarrhea at the same time.
