摘 要:目的比较金属垫块和颗粒植骨在全髋关节置换术治疗髋臼内上方骨缺损中的效果。方法将在河南省新乡医学院第一附属医院骨外科行全髋关节置换术治疗的86例髋臼内上方骨缺损患者采用随机数字表法分为金属垫块组和颗粒植骨组,各43例。金属垫块组采用金属垫块治疗,颗粒植骨组给予髂前上棘松质骨和人工骨治疗。比较两组的手术治疗效果、髋关节功能及手术相关指标。结果金属垫块组的手术治疗总有效率高于颗粒植骨组(P<0.05)。术后1、3个月,两组的Harris评分均高于术前,且金属垫块组高于颗粒植骨组(P<0.05)。金属垫块组的手术时间、恢复完全负重时间短于颗粒植骨组,术中出血量少于颗粒植骨组(P<0.05)。结论金属垫块治疗全髋关节置换术中髋臼内上方骨缺损的效果更好,能缩短完全负重时间,促进髋关节功能恢复。Objective To compare the the effects of metal pad and granular bone grafting in the treatment of bone defect inside and above acetabulum in total hip replacement.Methods Eighty-six patients with bone defect inside and above acetabulum treated with total hip replacement in bone surgery department of the first affiliated hospital of Xinxiang Medical University in Henan province were divided into metal pad group and granular bone grafting group by random number table method,with 43 cases in each group.The metal pad group was treated with metal pad,while the granular bone grafting group was treated with cancellous bone and artificial bone of anterior superior iliac spine.The effects of surgical treatment,hip joint function and surgical related indexes were compared between the two groups.Results The total effective rate of surgical treatment of the metal pad group was higher than that of the granular bone grafting group(P<0.05).At 1 and 3 months after surgery,Harris scores of both groups were higher than those before surgery,and those in the metal pad group were higher than the granular bone grafting group(P<0.05).The operative time and recovery time of full load in the metal pad group were shorter than those in the granular bone grafting group,and the intraoperative blood loss was less than that in the granular bone grafting group(P<0.05).Conclusion Metal pad in the treatment of bone defect inside and above acetabulum in total hip replacement is more effective,it can shorten the total load time and promote the recovery of hip joint function.
