摘  要:目的比较不同三维标测系统引导射频消融治疗特发性右室流出道室性早搏的临床效果。方法选取50例特发性右室流出道室性早搏患者作为研究对象,其中使用Carto三维标测系统引导射频消融治疗的21例分为A组,使用Ensite Array三维标测系统引导射频消融治疗的29例分为B组。比较两组的手术情况、手术效果、复发率及心肌指标。结果A组X线曝光时间显著短于B组(P<0.05)。术后3 d,A组CK、CK-MB、cTnT水平低于B组(P<0.05)。结论特发性右室流出道室性早搏采用Carto和Ensite Array三维标测系统引导射频消融均有较高的手术效果和较低的复发率,但Carto三维标测系统术中X线曝光时间明显更短,也有助于进一步减轻心肌损伤。Objective To compare the clinical effect of radiofrequency ablation guided by different three-dimensional mapping systems for idiopathic premature ventricular contractions orginating in the right outflow tract.Methods A total of 50 patients with idiopathic premature ventricular contractions orginating in the right outflow tract were selected as the research objects.Among them,21 patients using radiofrequency ablation guided by Carto three-dimensional mapping system were divided into group A,and 29 patients using Ensite Array three-dimensional mapping system were divided into group B.The operation condition,operation effect,recurrence rate and myocardial indexes of the two groups were compared.Results The exposure time of X-ray in the group A was significantly shorter than that of the group B(P<0.05).At 3 days after operation,the levels of CK,CK-MB and cTnT in the group A were lower than those in the group B(P<0.05).Conclusion Idiopathic premature ventricular contractions orginating in the right outflow tract using Carto and Ensite Array three-dimensional mapping system to guide radiofrequency ablation have higher surgical effect and lower recurrence rate,but the X-ray exposure time of Carto three-dimensional mapping system is significantly shorter,which also helps to further reduce myocardial damage.
