摘 要:目的探讨喙突下撞击综合征(CIS)与MRI测量参数之间的关系.方法采用1.5T MR测量并比较CIS(16例,病例组)和正常肩关节(35例,正常组)的喙肱距离(CHD)、喙突指数(CI)、小结节高度、喙突倾斜角有无差别.结果正常组中女性的CHD显著小于男性(Z=2.493,P=0.011).病例组的CHD为(5.67±1.76)mm,明显小于正常组(Z=3.279,P=0.001);病例组的CI为(19.99±2.72)mm,明显大于正常组(Z=3.258,P=0.001);病例组的喙突倾斜角为(18.19±4.43)°,明显大于正常组(Z=4.641,P<0.001);小结节高度在病例组及正常组间并无显著性差异.ROC曲线显示CHD、CI和喙突倾斜角诊断CIS的AUC分别为0.820、0.834和0.895.结论CHD、CI、喙突倾斜角对CIS的诊断有较好的效能,可以作为CIS的MRI定量诊断指标.Objective To investigate the relationship between MRI parameters and subcoracoid impingement syndrome(CIS).Methods The shoulders of 16 CIS patients(case group)and 35 healthy people(normal group)underwent 1.5T MR examination.The coracoid humeral distance(CHD),coracoid index(Cl),height of lesser tubercle and coraoid obliquity were calculated and compared between the two groups.Results The CHD of women was significantly lower than that of men in the normal group(Z=2.493,P=0.011).The CHD[(5.67±1.76)mm]of the case group was significantly lower than that of the normal growp(Z=3.279,P=0.001),and the CI[(1999±2.72)mm]and coracoid obliquity[(18.19±4.43)°]of the case group were significanly higher than those of the normal group(Z=3.258,P=0.001;Z=4.641,P<0.01).The height of lsser tubercle did not show statistical dflrence between the case group and the normal group.ROC curve showed the AUC of diagnosing case were 0.820,0.834,0.895 for CHD,CI and coracoid obliquity,rspectively.Conclusion CHD,CI and coracoid obliquity have good capacity for diagnosing CIS,and can be used as the quantitative MRI diagnostic index of CIS.
