摘  要:目的探讨基于单源双能量CT(DECT)低管电流扫描成像技术在痛风患者踝关节检查中的最佳扫描方案.方法选取临床高度怀疑痛风的患者90例,随机分为3组(其中低管电流分别设为120 mAs、100 mAs、80 mAs),行DECT非增强扫描及薄层(0.75 mm)重组.每位患者均行2次扫描,即常规管电流扫描(165 mAs)和低管电流扫描,记录容积CT剂量指数(CTDIvol)和平均剂量长度乘积(DLP)及图像的平均CT值、噪声,计算出SNR,并进行统计学分析.将原始数据输入syngo.via工作站中的痛风分析软件进行图像后处理,比较同组不同管电流扫描下图像质量有无差异.结果低管电流组120 mAs获得了较满意的DECT图像,低管电流组100 mAs及80 mAs所获得的DECT图像质量欠佳.运用低管电流(120mAs、100mAs及80mAs)扫描后所获得的CTDIvol和DLP与同组常规管电流扫描下获得的CTDIvol和DLP均有显著的统计学意义(P<0.05).结论低管电流单源DECT扫描较常规管电流扫描CTDIvol和DLP均有明显的降低,推荐管电流降低至120 mAs,减少辐射剂量,又能满足临床诊断的需求.Objective To investigate the optimal scanning scheme of single source dual energy CT(DECT)based on low-tube current scanning imaging technique in ankle joint examination of gout patients.Methods 90 patients with clnically highly suspected gout were randomly divided into three groups(low-tube currents were set at 120 mAs,100 mAs。and 80 mAs,respectively),and DECT non-enhanced scanning and thin slice(0.75 mm)recombination were performed.Each patient underwent two scans,a conventional tube current scan(165 mAs)and a low-tube current scan.The volume CT dose index(CTDIvol)and the mean dose length product(DLP)and the mean CT walue and noise of the images were recorded vand the SNR were calculated and analyzed statstically.The raw data was input into the gout analysis software in the syngo.via workstation for image post processing,and the image quality of the same group of diferent tube current scans was compared.Results The DECT images obtained by low-tube current group(120 mAs)were satisfactory。while those obtained by low-tube current group(100 mAs)and low-tube current group(80 mAs)were not.The CTDIvol and DLP obtained by scanning with low-tube current(120 mAs,100 mAs and 80 mAs)had statistically significant with that obtained by conventionak tube current scanning in the same group(P<0.05).Conclusion Compared with conventional-tube current scanning,CTDlvol and DLP of low tube current single source DECT are signifcant reduction.The recommended tube current is reduced to 120 mAs。which can not only reduce the radiation dose,but also fully meet the needs of clinical diagnosis.
